AFFIDAVIT OF CONSENT

WE, SPS. JAIME GOBOTIA & EVELYN GOBOTIA, Filipinos, both of legal
age, and residents of Purok 5, Bautista, Dasmarias City, Cavite, after being
duly sworn to in accordance with law, depose and state: That - -

1. That we are the biological parents of ELVIEVANE GOBOTIA who

was born on March 8, 2000 in Vinzon, Camarines Norte;

2. The said child will be travelling to Singapore for ASEAN SCHOOL

GAMES and will be accompanied by her coach FERNANDO DAGASDAS;

3. That we, as parents hereby give our full consent and conformity
to the said travel of our above-mentioned child to the said country.

4. We are executing this affidavit to attest to the truth and veracity

of the foregoing and for whatever legal purpose the same may serve;
