AFFIDAVIT OF DISCREPANCY

I, CARMEN P. RIVERA, Filipino, of legal age, and resident of Paliparan 3,

Dasmarias City, Cavite, after being duly sworn to in accordance with law, depose and
state: That - -

1. That I am a friend of JOESY ACEBIDO NUMERON who was admitted at St.

Paul Hospital located at Dasmarias City, Cavite;

2. Unfortunately, in her admission in the above said hospital, her name was
incorrectly registered as JOESY ACEBEDO NUMERON instead of
JOESY ACEBIDO NUMERON which is registered in her Certificate of
Live Birth and Philhealth.

3. Through this affidavit I hereby declare under oath her true and correct name
is JOESY ACEBIDO NUMERON.

4. Be it also known that the names JOESY ACEBIDO NUMERON and

JOESY ACEBEDO NUMERON refers to one and the same person.

5. I am executing this affidavit to attest to the truth and veracity of the foregoing
and for whatever legal purpose the same may serve;
