JOINT-AFFIDAVIT OF DISCREPANCY
WE, MARIETA C. YONGIS, Filipino, of legal age and resident of Blk 11, Lot 1,
Brgy Sta Fe, Dasmarinas, Cavite and MARK LESTER LOZARES, Filipino, of legal age,
and resident of Blk 27 Lot 4 Vill de Soleil , Dasmarias, Cavite, after being duly sworn to
in accordance with law, depose and state: That - -

1. We personally know NERISSA TEMPORAL SAEZ, widow being our

close family friends and neighbor for a long period of time;
2. We also know for a fact that she was born on 16 January 1968
at Zapote Bacoor, Cavite to Sps. Erlinda A. Temporal and Benjamin Temporal;

3. Unfortunately, when she secured a copy of the certificate of live

births in the National Statistic Office she found out that her name is incorrectly
registered as NERISA TEMPORAL instead of NERISSA TEMPORAL ;

4. Through this affidavit we hereby declare under oath that her true
and correct name is NERISSA TEMPORAL and also know that NERISA TEMPORAL
and NERISSA TEMPORAL was the same person ;

5. We are executing this affidavit to attest to the truth and veracity of

the foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 7th of
