JOINT-AFFIDAVIT OF DISCREPANCY

WE, GENE B. DEPOSITARIO, Filipino, of legal age and resident of Blk 89

Lot 19, Zone 9, Bautista, Sampaloc 4, Dasmarias City, Cavite and CARLITO A.
OLIGO, Filipino, of legal age, and resident of Brgy. Sampaloc 4, Dasmarias City,
Cavite after being duly sworn to in accordance with law, depose and state: That - -

1. We personally know RADIAN 1 ORONGAN DELA CRUZ as

the son of FLORENCIO BELLEZA DELA CRUZ and FLORITA LAYVA
ORONGAN being our close family friends and neighbor for a long period of time;

2. We also know for fact that he was born on MARCH 15, 1977 at
MANDALUYONG, METRO MANILA;

3. Unfortunately, when he secured his Certificate of Live Birth, his

MIDDLE NAME was incorrectly registered as ORONGON instead of
ORONGAN. Moreover, her mothers name was incorrectly registered in his
Certificate of Birth as FLORITA LEYVA ORANGON instead of FLORITA
LAYVA ORONGAN.

4. Through this affidavit we hereby declare under oath that his true
and correct name is RADIAN 1 ORONGAN DELA CRUZ and his true and
correct mothers name is FLORITA LAYVA ORONGAN. Also know for a fact
that RADIAN 1 ORONGAN DELA CRUZ and RADIAN 1 ORONGON DELA
CRUZ is one and the same person.

5. We are executing this affidavit to attest to the truth and veracity

of the foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 13th

day of June 2017 in Dasmarias City, Cavite.
