OF MS. KATHRYN BERNARDO

I, KATHRYN E. BERNARDO, 21 years old, single, residing at

Mendoza Compound, Lourdesville, BAlulang, Cagayan de Oro City , after
having been sworn to in accordance with the law do hereby depose and state:

That RHENFACEL J. MANLEGRO is the counsel who conducted

and supervised my examination as a witness at her office at Flores-Manlegro
Law Office, 3rd Flr., CKY Center, Capistrano St. Cagayan de Oro City;

That I am answering the questions herein fully conscious that I do so

under oath;

The following are the Questions propounded by Atty. RHENFACEL

J. MANLEGRO and my answers in English language:

Q1: Do you swear to tell the truth and nothing but the truth?
A1: Yes Maam.

Q2: Are you the same Kathryn Bernardo, witness in this case for Damages
now pending before the Regional Trial Court, Branch 41 of Cagayan
de Oro City?
A2: Yes Maam

Q3: Where were you on May 22, 2017 at about two oclock in the
afternoon?
A3: I was riding in a bus under the Reid Bus Line, Inc.

Q4: Is the number of the bus 8502?

A4: Yes, maam.

Q5: Where were you going?

A5: I rode in the bus from Malaybalay City, Bukidnon to Cagayan de Oro.

Q6: What transpired during that time?

A6: As I was riding in the bus, I noticed that I was the only passenger left
at 2:00PM. I initially wanted to stop at the Agora Bus Station.
However, when the bus stopped in the intersection of Osmea St. and
CM Recto Avenue, I saw a truck that turned right without slowing
down. Such truck eventually hit the bus I was riding.

Q7: What happened next?

A7: Because of the impact, I swiftly fell from where I was seated to
the ground. My lower back was painful and I had difficulty

moving but because I was already terrified that something worse

might happen, I endured the pain and went hurriedly out of the
bus.

Q8: Was the bus you were riding on stopped or was moving when it
arrived the intersection?
A8: The bus stopped. It was not moving that time. It was the truck
that was moving towards our bus.

Q9: Did you acquire any injury from the incident?

A9: Yes, because minutes after, police officers and the owner of the bus
line arrived to check on the vehicular accident. They assisted me to
the hospital so I will be checked. I got some bruises on my arms and
had lower back pain.

Q10: To which hospital were you brought to?

A10: I was brought to the Medical Hospital.

Q11: Did you personally see the driver of the truck?

A10: Yes I did. But I did not notice him that much because I was already
feeling dizzy. I was not able to talk to him.

Q11: Did the owner of the Busline Company assisted all your needs that
resulted from the incident?
A11: Yes, all the medical expenses were paid by them. I didnt pay a single
cost.

Q12: Do you have anything to say, Mr. Sales?

A12: No more, Maam.
