AFFIDAVIT OF SERVICE

April 2012 on Mr. Soon Hee Lee), Filipino, of legal age and with office
address at 3rd Floor, Libran House Building, 144 Legaspi corner V.A.
Rufino Steets, Legaspi Village, 1229 Makati City, hereby depose and
state that:

1. I am the messenger/ process server (kindly confirm his/

her Official Position/ Desiganation) of Valenton Gramata Loseriaga
Law Offices, with office address at 3 rd Floor, Libran House Building, 144
Legaspi corner V.A. Rufino Steets, Legaspi Village, 1229 Makati City.

2. As the messenger/ process server, I am tasked to serve

pleadings, letters and other written instruments on behalf of the
aforesaid law office (kindly confirm his/ her duties and
responsibilities).

3. On (date of service of the 30 April 2012 Demand Letter),

I served a copy of a demand letter dated 30 April 2012 (30 May 2012
Demand Letter) on Mrs. Soon Hee Lee (Mrs. Lee).

4. The 30 April 2012 Demand Letter was received by Mrs. Lee

(kindly confirm if the 30 April 2012 was received by Mrs. Lee herself
or by someone on her behalf).

5. I am executing this Affidavit to attest to the truth of the

foregoing and for whatever legal purpose it may serve.
