AFFIDAVIT OF QUITCLAIM AND WAIVER OF RIGHTS

I, ANITA OCO BALDOVINO, of legal age, Filipino, with

residence address at ____________________________, after
having been duly sworn to in accordance with law, do hereby
depose and state: THAT-

1. I am married to the late SARGEANT MARLOUN V.

BALDOVINO on ___________, however, we extra-judicially
separated for Fifteen (15) years prior to his death on __ May
2017.

2. I gave birth for two (2) children named: ___________

and MARIAN O. BALDOVINO;

3. When we separated, _______________ is in my custody

while I decided to left Marian O. Baldovino in the custody of
my parents-in-laws due to financial problems;

4. After the death of my husband, I found out that he

has one (1) illegitimate child named MARLOUN BALDOVINO JR.
and that the same is not an issue anymore considering the
number of years we separated;

5. The Philippine government provides monthly pension

to the beneficiaries of the murdered soldiers;

6. I am aware and recognize that my mother-in-law,

CORAZON V. BALDOVINO was the beneficiary indicated by my
husband in all his legal documents;

7. Anent to this, I do not interpose nor object to the

said declaration. I do not intend also to claim or submit
necessary documents for I live it to my mother in law subject
to the precondition that all the three (3) must receive equal
amount from the said pension.

8. I am entitled to receive money from my mother-in-

law in behalf of my daughter _____________________ and by
such consideration and/or agreement, I do hereby quitclaim,
release, discharge and waive any and all actions of whatever
nature, expected, real or apparent, which I may have against
any of the pending applications of my mother-in-law;

9. I declare that I have read this document and have

fully understood its contents. I further declare that I
voluntarily and willingly executed this Release, Waiver and
Quitclaim with full knowledge of my rights under the law.
