The Educator release form is for the release of the TKES/LKES/Contributing Professional in the
GaDOE TLE Electronic Platform from one local education agency (LEA) to another. The Educator
should understand that completing and signing the form will mean that the receiving LEA will have
access to their prior TKES/LKES/Contributing Professional plans. The Release Form is located on the
TLE Electronic Platform in the User Guides.
1. Educator completes and signs the form and submits to Human Resources (HR) in the new
county.
2. If a teacher decides to release their account, DO NOT create a new account within your
district. Wait until the account has been released and modify the information to match your
district setup.
3. HR in new county sends a scanned signed copy of the release form in an email to the Departing
LEA so that the Departing LEA can create PDF copies of all archived plans for their files.
4. HR in the Departing LEA saves all archived files (retrieve PDF file from the Summative
Assessment Container) needed for HR records. Then the LEAs district Super User releases the
account by changing the location to NO LOCATION (name may change in the future). The
Super User sends an email to the Receiving LEA (new district) to notify them that they have
released the account.
5. Receiving LEA HR informs the Receiving LEAs District Super User to update the account as
follows:
a. Remove the Departing LEAs 3 digit district code and add the Receiving LEAs 3 digit
district code.
b. Add the employees new location.
c. Update Account Roles and Rights as needed
6. Educators who leave a school district but do not complete a release form, the Educators
Account should be placed in the HR file by the District Super User. Districts are accountable
for making sure the HR files reflect Educators who are no longer with the district, but did not
release their account. This enables the Educator to access their account in their former school
district.
Release Form
The following educator is requesting the release of the TKES/LKES/Contributing Professional Plan in the GaDOE TLE
Electronic Platform from one local education agency (LEA) to another. The educator understands this will mean the
receiving district will have access to their prior TKES /LKES/Contributing Professional Plans.
(Optional) Super User Name and Email for departing LEA (Optional) Super User Name and email for
Receiving LEA
Please note that all plans associated with the account will be released. Indicating the year will help speed up the process
for departing district finding the plans to make copies of for their records. It is the departing districts responsibility to
make sure they capture all information before releasing a plan, even if it is not marked on the release form.
_____1. Departing LEA will need to have a release for each employee requesting the transfer of the TKES/LKES
Plan to the receiving LEA.
______2. Departing LEA will remove the location from the employees account and any admin rights associated
with the account if applicable.
Departing LEA may want to consider retaining an electronic file or hard copy of the
TKES/LKES/Contributing Professional plan prior to releasing it to the receiving LEA.
______3. Receiving LEA will retrieve the account in the GaDOE TLE Electronic Platform and assign the new
location and admin rights if applicable. The username will also need to be updated to include the receiving LEAs
3 digit code.
