02. Point no.6.2 Empanelment of Health care providersl

Minirnum one private hospital would be empanelled per 20,000 enrolments in every district. Criteria
for enrohnent may be relaxed by State Nodal Agency on case to case basis as indicated in tender
document page no.20.

03. Point no. 6.13 Assessment of Health care provider;

Each empanelled hospital be evalr:ated for quality assessment once in year as per the format
will
provided by the SNA.Failure in qLrality assessment may cause de- empanelment of hospitals.Refer
