INTRODUCTION

A. Background

In Egypt the mortality rate of liver cirrhosis patients was 41.6%. It is

important to assess the nutritional status of this group of patients and to
design a nutritional regimen because these patients suffer from protein
malnutrition. Based on research that has been done has a goal is an
assessment of the impact of nutritional regimens designed in patients with
liver cirrhosis. Adult patients with liver cirrhosis as a study group and 20
patients as a control group, the study was conducted in the Tropical
Medicine and Gastroenterology Department at Asyut University hospital.
The tools used for data collection are assessment sheets of patient
assessment, and Development of nutritional regimen. The result is the
most common malnutrition risk factors are dry mouth, taste changes, food
intolerance, some medications and the presence of ascites. The number of
patients who did not suffer from malnutrition increased due to the
implications of the nutritional regimen one and three months later. From
the research that has been done to get the conclusion that the application of
nutritional regimens for patients with liver cirrhosis showed an increase in
nutritional status of patients. Nutritional support and regimens, suggestions
and guidelines by dietitians should be performed for all cirrhotic patients
to prevent malnutrition complications and improve clinical outcomes.

DISCUSSION

A. Introduction
Liver cirrhosis is a chronic hepatic disease characterized by diffuse
destruction and fibrotic regeneration of hepatic cells. As necrotic tissue
yields to fibrosis, this disease alters liver structure and normal
vasculature, impairs blood and lymph flow, and ultimately causes
hepatic insufficiency [1,2]. The incidence of cirrhosis is increasing in
the developed world, mainly because of the life style factors which
lead to its development. According to the World Health Organization ,
there is about 3% of the world's population is infected with Hepatitis C
virus (HCV) and that there are more than 170 million chronic carriers
who are at risk of developing liver cirrhosis and/or liver cancer. More
than 240 million people have chronic (long-term) liver infections.
More than 780 000 people die every year due to the acute or chronic
consequences of hepatitis B. [4,5]. Available studies indicated that
liver cirrhosis developed in 4 % to 24% of persons after 20 years of
infection with HCV. These estimates of cirrhosis risk are influenced
strongly by the population studied and cohort recruitment methods [6].
Hepatitis C virus (HCV) is recognized as a major threat to global
public health, especially in Egypt which has possibly the highest HCV
prevalence in the world; 10% - 20% of the general populations are
infected. HCV is the leading cause of liver cirrhosis, hepatocellular
carcinoma (HCC) and chronic liver disease in this country.

B. Evaluation
I think Liver Cirrhosis Impact of Nutritional Regimen Patient
Outcome" is very useful for the reader including me. In the journal
explains a lot about cirrhosis of the liver from the beginning of
understanding, signs and symptoms, the study also describes the
population of developing countries affected by cirrhosis of the liver,
and which is primarily in terms of nutrition. The authors also write
about common factors that occur in patients with liver cirrhosis, one of
which is the lack of food intake and the application of catabolic levels.
The authors also tell how the right diet for liver cirrhosis patients and
described also the benefits, as well as vitaminapa The authors also
wrote the role of nurses in dealing with patients with cirrhosis of the
liver by knowing the subjective data. In the journal I think it is quite
good and complete and very useful for readers, especially in patients

with liver cirrhosis or family of patients who are suffering from liver
cirrhosis.

C. Interpretation
In my opinion, the journal about Liver Cirrhosis: Impact Of
Nutritional Regimen On Patients Outcome, the contents are quite
complete. In this paper, the authors add research results from various
studies such as the World Health Organization, and make their own
research that gets the result that most of the patients are women in their
fifties, such as on marital status, the majority are married. The results
of this study are supported by the assessment sheet and the socio-
demographic characteristic table listed in the journal. In this journal
also added images so that readers understand how anthropometric
measurement. It's just that this journal does not explain about the liver,
such as liver function and structure that exist in other journals namely
Assessment of Dietary Management of Patients with Cirrhosis Liver of
Shadia Mohamed Idris and Ebtesam AL Ali which includes the
structure and function of the liver.

D. Summary
Liver cirrhosis is a chronic hepatic disease characterized by diffuse
destruction and fibrotic regeneration of hepatic cells. As necrotic tissue
yields to fibrosis, this disease alters liver structure and normal
vasculature, impairs blood and lymph flow, and ultimately causes
hepatic insufficiency . The most common risk factors for malnutrition
are dry mouth, taste alteration, food allergies or intolerance, multiple
medications and the presence of ascites. Application of the nutritional
regimen for patients with liver cirrhosis showed an improvement in
patient's nutritional status. The majority of patients with decompensate
liver cirrhosis were malnourished regardless the etiology of liver
cirrhosis, however, the minority of patients were at risk for developing
malnutrition the results show the numbers of patients who didn't suffer
from malnutrition increased post implication of the nutritional regimen
one and three months later.

CLOSING

A. Conlussion
The most common risk factors for malnutrition are dry
mouth, taste alteration, food allergies or intolerance, multiple
medications and the presence of ascites. Application of the
nutritional regimen for patients with liver cirrhosis showed an
improvement in patient's nutritional status. The researchers found
that, the majority of patients with decompensate liver cirrhosis
were malnourished regardless the etiology of liver cirrhosis,
however, the minority of patients were at risk for developing
malnutrition the results show the numbers of patients who didn't
suffer from malnutrition increased post implication of the
nutritional regimen one and three months later.
B. Recommendation
Dietitian of the nutritional regimen should give nutritional
support, advice and guidelines to prevent occurrence of
complications of malnutrition and improve clinical outcom for all
cirrhotic patients.
