FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs EMPLOYMENT

AGREEMENT___________________________________________________

We are pleased to offer you the position of FULL TIME LECTURER WITH FATIM AZ
ZAHRA-FAZ VOCATIONAL ACADEMYs and your employment will be subject to
the following terms and conditions:-

1. CONTRACT PERIOD

Your commencing period is from 1st AUGUST 2017 untill you request to resign or any
ISSUE.
Will term and condition.

2. COMPENSATION

Your commencing basic salary will be:

Ringgit Malaysia: TWO THOUSAND FIVE HUNDRED only (RM 10000.00)
with allowances for telephone, Internet and transportations of:
Ringgit Malaysia: TWO Hundred only (RM 200.00)
and BONUS
Ringgit Malaysia: RM10 BONUS INCREASING MONTHLY only (RM 10.00)

*WITH TERM AND CONDITION IN STAFF DECLARATION

after three months probation subjected to such lawful deductions for Employees Provident
Fund, SOCSO and any other deductions as and when required by law.

Employees Provident Fund and SOCSO will be paid after the first month of service in
FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs
Salary, allowances and overtimes adjusted will be based entirely upon our assessment of
your progress and the contribution you make to FAZ Academy. Your salary will be paid to
you before 7th of next month.

3. WORKING HOURS

Your working hours will be as follows:-

Mondays-Fridays : 9.00 a.m. to 6.00 p.m. (one hour of lunch time)OR PRAYER at
1.35pm for zohor and 4.45 pm for solat asar
Saturdays/Sundays* : will be calculated separately as Overtime (*if needed)

The above is the FAZ Academy's official working hours. Working hours are subject to be
changed. You may be requested to be on duty on the FAZ Academys projects or events
beyond your working hours; and on your own initiative to work extra hours to meet your
work load as and when the need arises.

4. PUNCTUALITY

You are required to report for duty punctually at all times.

5. DUTIES AND RESPONSIBILITIES

You are required to follow the instructions/directions as set by FATIM AZ ZAHRA-

FAZ VOCATIONAL ACADEMYS

During the continuance of your employment:-

i. you shall comply with the policies and regulations as laid down by
from time to FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs ;

ii. you shall faithfully and diligently serve the FAZ Academy and perform your duties
to the best of your ability and not do anything which may jeopardize the interest;

iii. you shall not reveal any trade secrets and any confidential information of the of any
other part FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs y made
available to you or in connection with the except when d FATIM AZ ZAHRA-
FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs D isclosure is
necessary for the conduct of the FAZ Academy's business;

iv. you shall use your best endeavors to promote and advance the interest of FAZ
Academy and shall not conduct any personal business in any way or for any
purpose whatsoever for any part of your time during office hours, by or with any
person or firm other than FAZ Academy.

6. ANNUAL SALARY REVIEW AND BONUS

The increment and payment of bonus are not contractual or mandatory and are strictly at
the sole discretion of FAZ Academy. The increment and payment of bonus of minimum

RM10.00 increasing monthly, if declared by , shall be depend on FATIM AZ ZAHRA-

FAZ VOCATIONAL ACADEMYs :-

by the Management on a year-to-year basis.

7. ANNUAL LEAVE

You will be entitled to Fifteen (15) days per year of annual leave with full pay for calendar
year upon completion of twelve (12) months of continuous service.

Leave can only be taken subject to approval from and must be su bmitted to FATIM AZ
ZAHRA-FAZ VOCATIONAL ACADEMYs at least seven (7) working days in
advance of commencement of leave.

It is the responsibility of the employee to confirm whether the leave application has been
approved by the Department Head prior to commencement of leave.

8. MEDICAL LEAVE

You will be allowed medical leaves in accordance with the Employment Act, 1955 of
Twelve (12) days per year on condition that the leaves must be recommended by panel
doctors.

Medical leaves must be supported by valid medical certificates.

9. MEDICAL BENEFITS

All confirmed employees will be given free medical attention and treatment by Takaful
Hong Leong (worth RM200/monthly); approved by FAZ Academy.

Each treatment will be depending on policy limit amount and all the term and conditions
within medical policy.
All medical treatments must only be performed in panel hospital and/or clinic only.

10. HOSPITALIZATION LEAVE

Hospitalization leave will be in accordance with the provisions of the Employment Act
1955. As such, in the event hospitalization is necessary, the amount of paid sick leave can
be extended by up to sixty (60) days per calendar year.

11. PUBLIC HOLIDAYS

You will be entitled to paid public holidays on Government gazette public holidays as
stipulated in the Employment Act 1955 and which are recognized by . FATIM AZ
ZAHRA-FAZ VOCATIONAL ACADEMYs

12. EPF & SOCSO

Your contribution to the Employees Provident Fund (EPF) is obligatory at the prevailing
rate of 8% of your monthly salary. The Company will similarly contribute its share on your
behalf to EPF at the rate of 13% of your monthly basic salary. The rates of contribution
would vary according to the existing regulations governing EPF.

You will be required to contribute to SOCSO in accordance with the statutory rates
requirements.

13. TERMINATION OF EMPLOYMENT

Termination of employment after confirmation will require three (1) months notice in
writing or payment of three (1) months' salary in lieu thereof except in the case of dismissal
by FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs on the ground of
misconduct/breach of trust.

Upon cessation of your employment with FAZ Academy, you shall surrender to all
document FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs s, data or other
confidential information pertaining to the business of obtained or ma FATIM AZ
ZAHRA-FAZ VOCATIONAL ACADEMYs de by you during the course of your
employment with FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs.

14. RETIREMENT AGE

The retirement age for all employees shall be upon attaining the age of 55 years for male
and upon attaining the age of 50 years for female.

FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs may at its discretion transfer

or reassign you to another job position or from one section, division, department or location
to another within the company, wherever located based upon the needs of
FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs.
Each transfer may/may not entitle for salarys increment until further notice.

16. CONFLICT OF INTEREST

You are required to devote your whole time, attention, energies and skills solely to the
operation, business and management of and you shall FATIM AZ ZAHRA-FAZ
VOCATIONAL ACADEMYs not involve with any business that directly or indirectly
materially interferes with FAZ Academy.

17. CONFIDENTIALLY

All information obtained in the course of work with shall be FATIM AZ ZAHRA-FAZ
VOCATIONAL ACADEMYs deemed to be strictly confidential. No such information
shall be divulged to any third party, or use for any purpose other than in the performance of
your duties.

You must undertake not to actively engage in any other business while you are employed
with FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs unless written
permission is obtained from the Directors. You must specially guarantee not to reveal any
findings or confidential information which you come into contact with in the course of your
work in FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs

18. DRUGS

FATIM AZ ZAHRA-FAZ VOCATIONAL ACADEMYs will not tolerate employees

who take drugs nor are drug traffickers. It is also a condition of service that should you
found taking or traffickers or are addicted to drugs, or should you be convicted of any
offence under the laws pertaining to drugs at any time, you shall be liable to immediate
dismissal.

19. RULES AND REGULATIONS

You will have to abide by the rules and regulations of as well as those FATIM AZ
ZAHRA-FAZ VOCATIONAL ACADEMYs e staff circulars that may be issued from
time to time.
