APPLICATION FOR SEARCH WARRANT

COMES NOW, the undersigned SPO1 Melbert Aguilar presently assigned

with the Station Investigation and Detective Management branch San Mateo Rizal
and having been duly sworn to in accordance with law. Do hereby dispose and state
the following under oath:

THAT he was informed and verify believes that above-name respondents are
keeping in their possession and control at the premises they are occupying located
at the 125 Sampaguita St. San Mateo Rizal, the following laboratory described with
many sacks of chemicals, assorted paraphernalia and other equipment use or
intended to be used in the manufacture of different types of illegal drugs, to wit;

That the above facts was confirmed to him by his witness Police Officer SPO1
Aguilar, SPO1 Perez and SPO1 Gacho who were able to gain entry into the affair
mentioned premises of the respondents.
 THEREFORE, the undersigned phrase to this Honorable Court to issue a
search warrant authorizing him and /or his men to search the premises and if
machines are attached to the ground, padlock the premises described in the
application and to seize and bring to this Honorable Court the above mentioned
items to dealt with in a fall accord with acting laws.
