When submitting your completed and signed form, be sure to incorporate the last page (page 5 of
5) which willcontain your information in barcodes.

Please Note: If the surname is blank on the passport, the applicant has to fill the given name on
surname's column & first name column has got to left blank.

Photo Specification: 2 recent passport size colored images with matt or semi matt end, 60%-80%
face coverage, white background and while not border (Size: 35mm x 45mm)
Please note
a) Mention the name and date of birth at the back of the pic.
b) images mustn't be more than 3 months recent, scanned/stapled and may not be used in any of the
previous visas.

Covering Letter: Covering-letter from the applier on companys letter head stating his name,
designation, passport number, purpose and duration of visit punctually signed
by authorized signatory and addressed to The Visa Officer, Canadian High Commission, New
Delhi/ consulate General of Canada, Bangalore.

Invitation Letter:
a) invitation Letter from Canadian host / inviting Company on its letter head stating the aim of travel
and duration of stay and should be punctually signed with signatorys name and designation.
b) Proof of ongoing business activities between Canadian company and Applicant: Business
Correspondence, Contracts between applicant/ applicant's business, and Canadian
business, shipment orders/invoices/bills of lading for goods sold .

Ticket: Itinerary
 Financials a) Company and private ITR for three Yrs.
b) Six months earnings slip.
c) Company and personal financial statement for last six months
d) credit card copy/Foreign Exchange endorsement/Travelers Cheque
Note: so as to confirm timely process you must include a one page outline of
the financial info you're submitting.

Proof of Occupation:
a) Company Registration Certificate / Certificate of Incorporation.
b) transient Company Profile
c) Articles of memo (In case the applicant is MD/Director)
