APPLICATION FORM

StarGl!tz 3.0 (the Competition) aims to discover potential singers in Malaysia and to provide an
opportunity for the talents to materialise their dreams by tying them with renowned recording labels. If you
think you have amazing voice and have a burning desire to materialise your dream, please fill out this
application form as per the requirement below.

ELIGIBILITY:

Malaysian citizen;
Aged 16 and above;
Contestant can be solo or in a group of people; and
Choice of song can be own original materials or cover songs.

ENTRY PROCEDURES & CONDITIONS:

Contestant is required to sing a song with a minimum duration of 60 seconds to a

maximum of 90 seconds, recorded into a video clip and upload to YouTube. The privacy of the
uploaded video shall be set to unlisted. (Please see YouTube video privacy settings for more
information);

Any video which was previously published in any forms and platforms cannot be used
for this Competition and application;

The uploaded video shall be named as StarGl!tz 3.0 followed by Name of the Song
& Contestants/Group Name.

Contestant must complete the Application Form which can be downloaded from
www.mycreative.com.my;

The link of the YouTube video, duly completed application form and a photocopy of
NRIC of Contestant(s) shall be sent to starglitz3.0@mycreative.com.my by 30 September 2017;

The link of the unlisted YouTube Video shall not be shared or sent to any third party
save and except for this Competition;

The subject of the email should be StarGl!tz 3.0 followed by the

Contestants/Group Name;
 The Contestant(s) shall only submit one application;

Each application will be acknowledged by an auto-receipt reply after it successfully

reaches MyCreative mailbox. If the contestant does not receive any reply within one (1) week of
submission, kindly contact starglitz3.0@mycreative.com.my; and
Application that fails to comply with any of the above Entry Procedures and
Conditions will be disqualified.

SPECIAL TERMS AND CONDITIONS:

Where an original material is used or performed by the Contestant during any stages of the Competition, the Contestant must ensure and undertake that it
does not infringe any rights, ideas, or concepts of third party;

Where a Contestant is selected as the Top 10 Finalists of the Competition, one must be able to take time off from their daily activities, or employment at any
time during the Competition period, as may be requested by the Organiser at its sole discretion;

In the event the Contestant decides to withdraw from the Competition, the Contestant shall, on demand, reimburse the Organiser for all losses, costs and
expenses incurred by the Organiser as a result of the Contestants withdrawal (including without limitation fully refunding any payments made by the Organiser to the Contestants);and

The Contestant shall notify the Organiser of any contractual obligations or agreements entered with any recording labels and the Organiser reserves the right
to request for such contracts / agreements.

GENERAL TERMS AND CONDITIONS:

By entering this Competition, Contestants agree to comply with Entry Procedures and Conditions, General Terms and Conditions and Special Terms and
Conditions (collectively called Terms and Conditions). Non-compliance of these Terms and Conditions will result in disqualification from the Competition;

The Contestants must adhere to the mechanism of the Competition as may be briefed and communicated to the contestants by the Organiser throughout the
Competition;

By entering this Competition, Contestants hereby warrant that all information submitted are true, current and complete. The Organiser reserves the right to
verify the eligibility of all Contestants and disqualify a contestant for submitting false information;

By entering this Competition, Contestants hereby irrevocably and unconditionally consent and authorise the Organiser to use its name, likeness, image, voice,
and/or appearance as such may be embodied in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made;

By entering this Competition, Contestants must agree and understand that the Organiser shall have complete ownership of such pictures, photos, videos,
recordings, audiotapes, digital images, and the like, taken or made on behalf of the Organisers activities and programmes, including the entire copyright, and may use them for any
purpose consistent with the Organisers vision and mission that shall include, but are not limited to, illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications,
advertisements, and any promotional or educational materials in any medium now known or later developed, including materials on the internet;

By entering this Competition, Contestants must acknowledge that it shall not receive any compensation, etc, for the use of such picture, photos, video
recordings, audiotapes, digital images, and the like, take or made by the Organiser, and hereby release the Organiser and its agents and assigns from any and all claims which arise out
of or are in any way connected with such use;

The Organiser reserves the right to cancel or amend the Competitions rules at its sole discretion without any prior notice. Any changes will be an amendment
to the mechanism and Terms and Conditions of the Competition;

The Organiser reserves the right to disqualify any Contestant if it has reasonable grounds to believe the contestant is not eligible or has otherwise breached
any of the Terms and Conditions;

The Organiser reserves the right to request the Contestant to the change Contestants song for the Competition as the Organiser deems fit;
 The Organiser reserves the right to amend, postpone or reschedule the Competition period or any of the dates thereof as its sole discretion;

The Organiser reserves the right to showcase/publicise/disclose the video/details included in the application for public viewing for public voting or any other
purposes deemed appropriate by the Organiser and the Organiser reserves the rights to brand, tag, mark, edit, modify, extract, impose any changes to the video as deemed appropriate
for the abovementioned purpose; and

In the event of any dispute regarding the rules, conduct, results and all other matters relating to the Competition, the decision of the Organiser shall be final
and no correspondence, discussion and/or appeals will be entertained at any point of time throughout the Competition period.

ORGANIZERS LIABILITY:

Any problem, loss, injury, disappointment or damage of whatsoever nature suffered by any party due to any delay and/or failure in submitting the application
and throughout the Competition period;

Any problem, damage or loss of whatsoever nature to any Contestants, or their authorised representatives resulting from their participation on the
Competition at any point of time throughout the Competition period;

Any compensation sought by third party as a result of copyrights infringement caused by any Contestants;

Any damage or loss of whatsoever nature to any Contestants music instruments, original materials, computer and/or electronic devices;

Any technical issues in relation to any telecommunications or internet network (including in relation to speed or bandwidth), including any injury or damage
to Contestant's or any other person's device related to or resulting from participation or downloading any materials in the Competition; and

Any other liabilities, actual or contingent that may arise from any other reasons and parties associated with this Competition and materials submitted.

PERSONAL DATA PROTECTION:

I/We agree that, with regard to the personal data, which are provided to MyCreative in this application form, I/We:

consent for MyCreative to process my/our personal data provided in this application form, and any other additional personal data which may be further
provided to MyCreative in connection with this application;

confirm that my/our personal data provided are true, complete and accurate;

acknowledge that I am/we are entitled to obtain access to and to request correction of the personal data provided to MyCreative and that I/We can request
MyCreative to cease processing my personal data. For this purpose, I/We should contact MyCreative, either by sending an email or by sending a letter to MyCreative at MyCreative

any of MyCreatives vendor(s), contractor(s) and/or outsourcing company(ies) engaged by MyCreative, for purposes in connection with this application and
any subsequent processing and/or transaction thereafter; and/or

Government authorities, agencies and and/or any other regulatory body, to the extent that the Government or other authorities, agencies and/or and other
regulatory body need to know, or as permitted by applicable laws and regulations.
I/We further agree that in any event MyCreative receives any request by any data subject for withdrawal of consent to process any personal data provided herein, MyCreative has the right to treat this
application as null and void.
