A point of fire origin could not be ascertained, however items of interest were tagged, documented, and

statements, and early photos and video of the fire event, put the fires origin in the area of the living room
of Unit #2602.

Fire Classification:
Based on the totality of this investigation to include but not ilmited to the physical examination of the incident
scene, observed fire patterns, photographs, witness statements, and the culmination of my training, education and
experience, it is with a reasonable degree of fire investigative certainty that this incident was caused by an
undetermined event. Items of interest were identified, tagged, and left in place for further analysis. See
Section 6, pages 9-18.

This flre?s cause has been classified as UNDETERMINED pending any further information and final forensic
analysis of tagged items.

Based on the recommendation of the U.S. Fire Administration, the International Code Councils Building Valuation
Data formula (March 2017) was used as a baseline for calculations. The 2017 DoD Honolulu Area Cost Factor was
used to adjust our estimate for Hawaii. This estimate includes damages caused by fire, smoke, water and necessary
overhaul:

Investigators returned on multiple dates in continuance of the fires origin and cause investigation.

Additional Comments:

5 IMMEDIATELY DANGEROUS TO LIFE AND HEALTH (I LH) EVALUATION

I performed a visual IDLH evaluation of the incident scene and observed concerns that could prevent me from
continuing my investigation into the Origin and Cause of this fire incident. Appropriate actions were taken to
ensure a safe investigation utilizing appropriate personal protection equipment (PPE) and with caution
regarding the observed hazard(s).

Cleaning of Personal Protective Equipment (PPE) and Tools


All personal protective equipment and tools utilized during the course of this investigation were properly cleaned
and decontaminated or disposed of at the conclusion of the on-scene phase of this investigation.

This was done in accordance to the Honolulu Fire Departments Policy & Procedures Manual (Chapter 4, Par. 4-
02.03.200 Inspection, Cleaning and Decontamination of Personal Protective Equipment), The Honolulu Fire

Departments Fire Investigation Standard Operating Guidelines (Article 13, Par. 5-13.02.300 PPE and Equipment)

and industry accepted practices for Fire Investigators outlined in the NFPA 921, 2017 edition, Guide for Fire and
Explosion Investigations (Chapter 13.6 Personal Protective Equipment).

6 OBSERVATIONS AT THE IDENTIFIED AND/OR PROBABLE POINT OF FIRE ORIGIN

The Structure was examined from the exterior to the interior and from the least fire damaged to the greatest fire
damaged in consideration of ventilation, fuel load, witness statements and my overall observations.

Witness statements are noted in Appendix A while any supplemental documentation is noted in Appendix B.
Digital photographs were taken at this incident scene and are in the case file.

The following observations are specific to the identified Area of Fire Origin. This is an example diagram copied
from the Marco Polo building plans. It is a representative sample of unit #2602 and other two (2) bedroom
units throughout the building. The Marco Polo diagram was used because it incorporates the electrical
schematic.

According to the occupants (Mr. Rockett) observation, the fire started in the approximate area
surrounded by the circle below. A plume of smoke emanated from the floor and shot up to the ceiling
followed by what sounded like a whooshing sound, then fire appeared.

After review of our excavations and Mr. Rocketts statement, we identified evidence of two possible
ignition sources in the living room area. Those two sources were a MAPP gas cylinder with torch head and
a lighter wand which were both tagged as items of interest. However, they were not in use prior to the fire
according to Mr. Rockett.

7 HYPOTHESES CONSIDERED

Natural Fire Cause(s):

- No severe weather activity was observed or reported in the area.

Level of Certainty: No supporting evidence to indicate a natural fire cause.

Accidental Fire Cause(s):

- Accidental fire causes were considered however all could not be systematically ruled out.

Level of Certainty: Possible, with a reasonable degree of fire investigative certainty.

Electrical failure/malfunction of a window mounted A/C unit (damaged by occupants).


Electrical failure/malfunction of a desktop computer (due to proximity to area of fire
origin).
Electrical failure/malfunction of a portable computer (to include battery failure).
Duplex receptacles with various anomalies (labeled A, B, D, E, and F).

Recommendation.
It is my recommendation that all items identified (tagged and un-tagged) in the area of origin, and/or at the
identified probable point of origin, be further examined by a certified forensics laboratory or other certified
authority in order to confirm or repudiate my final fire cause determination. Items are the responsibility of the
property owner and occupants and/or their representatives once the scene has been released by the Honolulu Fire
Department.

10.1 Fiiu~ CREW STATEMENT/OBSERVATIONS CAPTAIN Duici~ ROBINS

Name: Duke Robins. Rank: Captain.

Assigned Fire Company: L29/2I~d Platoon.

Role at Incident: Fire suppression.

Location of the Interview: Station 29, 2425 Date St., Honolulu, HI. 96816.

The following was related to me by Captain Duke Robins.

Captain Robins and his crew were in the fire station, Station 29, when a civilian came and notified them of
smoke coming from the upper floor of the Marco Polo building. They went to the corner of Date St. and
University Ave. and saw black smoke emanating from about the 20th floor of the Marco Polo.

Captain Robins then called the Fire Communications Center (FCC) to see if anyone reported a fire at the
Marco Polo, but FCC said no. He then told them that there was a working fire going on at the Marco
Polo. He did not tell FCC that his company was responding.

At the same time, Ladder 2 was refueling when the call came in from FCC assigning Ladder 02 to the
structure fire at the Marco Polo. Ladder 2 responded.

Captain Robins told FCC that L29 was at scene.

Ladder 29 parked just short of the ewa loading dock.

Captain Robins and his crew entered the Marco Polo from the front lobby and observed smoke coming
out of the elevator shafts in the lobby.

Captain Robins expected to get a ride from the lobby elevators but they did not work. Captain Robins saw
people walking out of the elevators, then shortly after heard that the guest/fire service elevators were out
of service. The elevators werent working. Ladder 29 was the first company to get to the fire floor via the
freight elevator. Ladder 02 had taken lobby control.

Captain Robins was not sure if the elevator doors opened in the lobby.

It took approximately 3-4 minutes to get from the guest elevators to the freight elevators.

No one had freight elevator control. Ladder 29 had security take their crew to the freight elevator. It is a
small freight elevator and on the way up, lights in the elevator acted erratically. Ladder 29, Rescue 01, and
Engine 03 went up the freight elevator until it failed. Each of the companies went up one crew at a time.

Once Ladder 29 got to the fire floor, the crew hooked up to the standpipe and tried to make entry to the
26th floor, but it was too hot. Ladder 29 opened the door for about 10-15 seconds, then closed it.

Ladder 29 changed out SCBA cylinders several times before Engine 03 arrived.

Captain Robins observations on arrival to the incident site included:

- Fire was coming out of the apartment window and not the air conditioning vent.

- Smoke was coming from the lobby elevator shafts at the ground level.

- Building occupants were slowly coming down from floors above the lobby.

- There were reports of occupants being stuck in their apartments.

- Ladder 29 apparatus did not sustain any major damage from falling debris.

10.2 FIRE CREW STATEMENT/OBSERVATIONS CAPTAIN RAYMOND EG~r

Name: Raymond Egan. Rank: Captain.

Assigned Fire Company: Ladder 02/2nd Platoon.

Role at Incident: Fire suppression.

Location of the Interview: Telephone conversation. Captain R. Egan was in quarters at Station 2, 1610
Makaloa St. Honolulu, HI. 96814.

The following was related to me by Captain Raymond Egan.

Captain Egan and his crew responded from Station 29 and reported a fully involved structure fire at the
Marco Polo building. Ladder 02 arrived before Ladder 29.

Upon arrival, Captain Egan and crew secured lobby control, with the fire being reported on the 26th floor.
Security personnel were in the lobby alerting residents of the fire. Non-ambulatory residents were assisted
by other residents coming down the stairwell.

Heavy black smoke was filling the lobby area from the elevator shafts.

Elevator #1 appeared to have been recalled but did not open. Captain Egan observed the by-pass panel at
the #1 elevator hanging by its wires. There was also black tape across the key hole of the by-pass plate. It
appeared the black tape was holding the by-pass plate in place.

Eventually Captain Egan got into the #1 elevator but could not get it into phase 2 (manual control of the
elevator).

Security personnel took Ladder 02 personnel to the freight elevator. Ladder 29, Rescue 01, and Engine 03
used the freight elevator to get to the 24th floor. This worked for approximately six (6) trips. The elevator
was small and could only transport one crew at a time.

Captain Egan went back to the lobby elevators where a single elevator worked for one trip, then failed.

Otis elevator representatives arrived on scene to fix the elevators and were able to shuttle HFD personnel
and firefighting equipment to the staging area.

SCBA bottles and high rise equipment were also shuttled up the ewa stairwell.

10.3 FIRE CREW STATEMENT/OBSERVATIONS CAPTAIN ALVIN KU

Assigned Fire Company: Engine 03/1st Platoon.

Role at Incident: Fire suppression.

Location of the Interview: Station 31, 1334 Nimitz Hwy., Honolulu, HI 96817.

The following was related to me by Captain Alvin Ku.

Captain Ku is assigned to E31/3~, but was relieving as Captain on Engine 03 the day of the fire.

Ladder 29 was first to try to make entry to the fire floor on the ewa end, but were unsuccessful.

Engine 03 relieved Ladder 29 on the fire floor and tried a second time to go around the corner stairwell to
the 26th floor. The smoke and heat made entry to the fire floor untenable. Because of the heat and smoke,
Captain Ku was afraid for his crew and told them to back out. Engine 03 personnel could not do anything
or could not get near the fire. Engine 03 personnel could not open the door to the 26th floor. Engine 03
personnel shot water on the door and steam came off of it. At one point, Captain Ku opened the door to
the 26th floor and could see fire down the hallway.

There was no staging at this time, they couldnt get to the 27th floor. 1St alarm companies stayed on the 26th

floor.

Engine 03 personnel stayed on the fire floor.

On the third rotation, Engine 03 personnel went to the 28th floor.

On Engine 03s second cycle in, they found the first victim in Unit #2615 by kicking in the door and
making entry. Captain Ku radioed to command that one victim was found deceased. Engine 03 personnel
continued their search.

Moving toward Unit #2613, Captain Ku saw a glow stick on the ground fronting Unit #2613. Engine 03
personnel went in to Unit #2613 and found two victims. The glow stick was from Engine 25 personnel
marking apartments with fire related victims. This information was also related to command.

After changing out their SCBA bottles, Engine 03 personnel moved to upper floors to help extinguish fite
on the 27~ and 28th floors.

Later, Engine 03 personnel moved down to the 26th floor to look for any rekindled fire.

Engine 03 personnel included, FF3 Brad Chong and two Fire Recruits.

When walking throughout the hallway, Engine 03 personnel could not see a foot ahead of them. At one
point, Captain Ku was concerned about flashover.

Water was an issue coming up the #2 stairwell as there was no standpipe.

Before staging was established, Captain Ku observed that entry to the fire floor was attainable from the
middle stairwell (stairwell #2). The 26~ floor was somewhat clear but there was no way to get water at the
time.

On the 26th floor and in the hallway, Captain Ku observed fire extending from a mauka apartment and
moving in a horizontal fashion to the adjacent hallway wall. As they approached the #1 stairwell (ewa
stairwell) from the #2 stairwell (middle stairwell) of the 26th floor, he could see flames jetting sideways
from one of the apartments and impinging on the adjacent wall.

The following was related to me by Ms. Laura Gearhart.

Note: Attorney for Ms. Laura Gearhart, Scot Brower, was present during this interview.

Ms. Gearhart related that normal stuff went on the night before the fire. She was ordering stuff online
and her fiance Thomas Miller and his friend, Jeffrey Rockett, were in the garage working on the bike.

On July 14, 20127, at approximately 0200 hours, Mr. Miller went to unit #2602. Ms. Gearhart and Mr.
Miller fell asleep at approximately 0230 hrs.

Later that same day, Ms. Gearhart was woken up by Mr. Rockett at approximately 1415 hours. Mr. Rockett
entered her room saying there is a fire. Mr. Rockett told her to put a shirt on, we got to go now. It was
dark so Mr. Rockett told her to grab his foot as he led her outside the apartment. All Ms. Gearhart could
see was black smoke. While exiting the apartment, Ms. Gearhart bumped her head on the bed and scraped
her knee.

I asked Ms. Gearhart to describe what the smoke looked like. Ms. Gearhart related that the smoke was
very dark. When Ms. Gearhart came out of the room and entered the hallway toward the kitchen, the
kitchen was black/dark with smoke. There was no fire or heat, just smoke in the living room. The
apartment was thick with smoke and it was hazy, Ms. Gearhart related. In the living room, Ms. Gearhart
could see light but could not breathe. Ms. Gearhart saw the smoke but knew in her head that she could
not breathe. Ms. Gearhart related that she could not see anything, it was pitch black in the kitchen.

Ms. Gearhart related that she was worried about being sued and thought that the information released on
the news was irresponsible and was causing her distress. Ms. Gearhart wanted to know who the
investigator was. Ms. Gearhart was informed that Isaac Raphael was the investigator. Ms. Gearhart also
asked who the fire chief was. She was informed that Chief Manuel P. Neves was the Fire Chief of the
Honolulu Fire Department.

Ms. Gearhart added that she felt the time line, from time she woke up until the time she called her mom,
was odd because it normally takes her 15 minutes to get down stairs. Ms. Gearhart mentioned that at the
time of the fire, she got up at approximately 1415 hours and called her mom when she reached downstairs

at 1500 hours. From the time she woke up to the time she says she got downstairs it took approximately 45
minutes.

Mr. Rockett was the first out the entry door.

Ms. Gearhart does not remember if the entry door closed behind her.

Note: At 1214 hours, during this interview with Ms. Gearhart, she asked to speak with her attorney, Scot
B rower, privately.

Ms. Gearhart and her attorney returned at approximately 1217 hours and I resumed my interview of her.

I asked Ms. Gearhart if Mr. Miller normally closes the front (solid core) door or louvered door to her
apartment. Ms. Gearhart related that she cannot speculate. I asked if I could continue the question and
Ms. Gearhart related that my question was not really a question but that I could continue. Through my
questioning, I found that the louvered door was always kept open. Ms. Gearhart described it to be the type
of door that folds in the middle with a little door knob in the middle. Ms. Gearhart added that when the
entry door (solid core) is opened, the doors in both bedrooms slam shut, especially when the trade winds
blow.

Ms. Gearhart described the living room:

-The couch was cream in color and made of a fabric material, is soft, and is the type of couch you
sink into when sat upon. Ms. Gearhart and Mr. Miller bought it on craigslist. Ms. Gearhart is not
sure how old it is or when it was purchased.

- Ms. Gearhart and Mr. Miller recently rented a storage locker so there were no tools in the living
room, but that I would need to ask Mr. Rockett where tools in the living room might be. Ms.
Gearhart added, there might be a Dremel tool.

-The air conditioner was bought on Thursday (the day before the fire) but Ms. Gearhart added that
she needed to check her records.

- The television was recently hung on the living room wall (wall mount).

-The window blinds were white vinyl slat plantation style (horizontal blinds). Typically, the slats
were open in the living room

-The blinds were partially open in the middle bedroom and similar to the ones in the living room
and master bedroom.

- The master bedroom blinds were shut to keep sunlight out.

- Ms. Gearhart recently bought a light fixture that was placed in the south west corner of the living
room. Ms. Gearhart was changing from halogen to LED lamps (looking for different effects). LED

bulbs were in the lamp at the time of the fire. Ms. Gearhart could not recall if the lamp was on or
off.

- The lamp by the air conditioner was off.

Ms. Gearhart recalled that in January there was a problem with the lights ffickering in her apartment. The
neighbor in Unit #2601 related to her that he was experiencing the same thing.

There was no recent work done in the apartment other than replacing the air conditioner.

Ms. Gearhart usually leaves the hallway light on at night for the boys, but everything else is left off,
including- lights and electronic devices.

Ms. Gearhart was not washing clothes at the time of the fire, but did do a lot of laundry on the day before
the fire because she was going to donate some of the clothes. Ms. Gearhart does not recall if clothes were
left in the dryer after drying.

Ms. Gearhart related that the lamp in her room was probably on.

I asked Ms. Gearhart where she believed the fire started. Ms. Gearhart related that she thought the fire
started on the south side of the building and not in her apartment.

Ms. Gearhart related that she is very upset. Ms. Gearhart stated that this incident was worldwide and she
was upset that her apartment had been targeted. She also called the Mayor regarding the handling of this
incident.

Ms. Gearhart related that she lost her cat and three birds in the fire.

The following was related to me by Mr. Jeffrey Wade Rockett.

Note: Attorney Scot Brower was present during the interview. It was unknown to me whether Mr. Brower
represented Mr. Rockett.

Mr. Rockett had been homeless for approximately 2-1/2 years. Mr. Rockett came back to Hawaii three (3)
years ago when his wife died. Mr. Rockett was married for 19 years. Mr. Rockett and Mr. Thomas Miller
had been best friend since they were ten (10) years old.

Mr. Rockett had been living with Ms. Gearhart and Mr. Miller for about a week. Before that, Mr. Rockett
was sleeping in Mr. Millers truck, which was a 1995 or 1996 Pathfinder.

The night before the fire, Mr. Rockett was working on Mr. Millers Hyundai Tiburon in the garage
(assigned garage stall for unit #2602 is unknown) because he knew hQw to fix cars. Mr. Rockett worked
on the vehicle until the security lady told him to pack it up for the night, which was at about 0100 horns.

After cleaning up (approximately 0100 hours), Mr. Rockett left to get breakfast at McDonalds. First he
went to the McDonalds on Kapiolani Blvd. (he thinks it was Kapiolani Blvd.), but it was closed for
maintenance. So Mr. Rockett went to the McDonalds on Nimitz in Kalihi. Mr. Rockett used Mr. Millers
truck. Mr. Rockett returned to the Marco Polo at about 0200 hours in the morning. Upon Mr. Rocketts
return, Mr. Miller met Mr. Rockett downstairs in the garage where they both smoked cigarettes. They
both went upstairs at approximately 0230 hours.

Mr. Rockett fell asleep in the living room on the floor between the coffee table and the couch, and did not
shower before he fell asleep.

Mr. Rockett related that there was nothing unusual going on in unit #2602 before he went to sleep.

I asked Mr. Rockett if he went into the kitchen prior to going to sleep, Mr. Rockett related he did not.

On July 24, 2017, at approximately 1415 hours, Mr. Rockett awoke to the sound of what he described as an
alarm clock going off.

Mr. Rockett thought it was an alarm clock coming from Mr. Millers room, so he yelled for Mr. Miller to
turn off the alarm.

While lying down, Mr. Rockett observed a light ambient smoke in the air so he sat up and went to the
kitchen to see if something was burning on the stove. Mr. Rockett related that Mr. Miller sometimes cooks
on the stove.

Mr. Rockett related that there was no real smoke or nothing, just ambient smoke. When Mr. Rockett
got to the stove the fire instantly appeared. Mr. Rockett did not understand what was happening. It all
happened so quickly, there was no fire at first, just ambient smoke. And then there was fire.

Mr. Rockett related that he saw a plume of smoke shooting up from the floor to the ceiling in the middle
of the room about two feet away from the north wall. Then suddenly, the smoke turned to flame.

Mr. Rockett stated There was smoke in the air and instantly there was fire on.

Mr. Rockett describes the sound to be like a whoosh sound when he saw the smoke shoot from the floor
to the ceiling and then catching fire.

Mr. Rockett yelled for Mr. Miller and Ms. Gearhart, but Mr. Miller did not answer. Mr. Rockett went to the
Mr. Millers bedroom and shoved the door open. Mr. Rockett saw Ms. Gearhart halfway sitting up on the
bed but appeared to be still sleeping. Mr. Rockett told her, We gotta go, this place is on fire.

When exiting the apartment, Mr. Rockeit told Ms. Gearhart to grab his foot when they exited the front
door. As soon as they got out they were crawling. Mr. Rockett related that it was a crawl to the front
door.

Mr. Rockett did not feel Ms. Gearharts hand on his foot when he got on the floor so he jammed his foot
back to keep the door open until he felt Ms. Gearhart grab it then they crawled out. Mr. Rockett did not
think about closing the door behind them when they exited unit #2602.

When Mr. Rockett opened the entry door to unit #2602 to exit (the entry door was closed at the time), he
realized the alarm he was hearing was the smoke alarm from the elevator lobby on the 26th floor, not the
alarm clock in Mr. Millers bedroom.

Mr. Rockett stated, The hallway was dark as shit.

Mr. Rockett related he definitely saw fire near the middle of the living room. The smoke went up, hit the
ceiling, then the smoke pushed into the kitchen.

Mr. Rockett and Ms. Gearhart went out the entry door and turned left down the hallway. Mr. Rockett and
Ms. Gearhart went left because they always use the elevators. It was an instant call.

When Mr. Rockett opened the door to the hallway, Mr. Rockett and Ms. Gearhart were already on the floor
because there was so much smoke. It appeared to be the same amount of smoke in the living room and
the 26th floor hallway.

Mr. Rockett related that the smoke cleared up as they were going down the hallway toward the elevators.

Once Mr. Rockett and Ms. Gearhart got past the elevators, they got up and ran. Mr. Rockett and Ms.
Gearhart exited the #3 stairwell (Diamond Head side).

By the time Mr. Rockett and Ms. Gearhart got downstairs, the Honolulu Fire Department was already at
the scene. Mr. Rockett went to check to see if Mr. Miller was in the garage and Ms. Gearhart went to the
bottom floor. Within 2-5 minutes of Mr. Rockett reaching the garage, Mr. Miller showed up.

The door to unit #2602 was closed when Mr. Rockett was alerted to the fire.

Mr. Rockett saw a white haze in the living room and did not think it had an odor. Mr. Rockett inhaled
some smoke but did not recall getting a headache.

Mr. Rockett did not smell burning plastic and related he had a good sense of smell.

Mr. Rockett thought the fire started from the unit below because it looked like the fire was coming out of
the floor.

Mr. Rockett does not know how the fire started in the apartment if the smoke alarms were going off in the
elevator lobby.

Mr. Rockett related he did not have to kick Mr. Millers bedroom door open to get Ms. Gearhart.

Mr. Rockett did not see the house line across unit #2602, and related, It was solid black. He could only
see a couple of inches off the floor, which he described as, Four (4) to eight (8) inches tops.

Mr. Rockett related that he did not get any soot on his face or burns from heat, however, Ms. Gearhart
looked like she had on black lipstick. Mr. Rockett thought it was soot from the smoke.

Mr. Rockett related that the fire went straight up from the floor to the ceiling in the living room.

Mr. Rockett was 90% sure the smoke was an off white smoke color.

Mr. Rockett related that flame shot straight up to the ceiling then black smoke. As far as Mr. Rockett
could tell, the air conditioner was not burning.

Mr. Rockett was in front of Ms. Gearhart when they exited the apartment, and the louvered door was open.

Upon exiting the 26th floor, Mr. Rockett did not notice any other apartment doors open.

Mr. Rockett related that there was no smoldering of smoke and no open flames prior to the fire in unit
#2602.

Mr. Rockett related seeing just ambient smoke in the living room of unit #2602.

Mr. Rocketts defined ambient smoke as, A thin, white smoke. Not thick. Its like standing in fog. I could
see all the way through the smoke, like in a bar filled with cigarette smoke.

Mr. Rockett did not know that Mr. Miller left unit #2602.

Mr. Rockett does not recall if the lights were on in the living room at the time of the fire.

Mr. Rockett related that the carpet was cream colored with a throw rug.

I asked Mr. Rockett if anything was on the coffee table, he related that there was nothing on it because
Ms. Gearhart wanted to move the coffee table.

Mr. Rockett related that the coffee table had drawers and a sliding shelf, but the top was missing. Mr.
Rockett got it from downstairs at the big item Tuesdays.

Mr. Rockett related that inside the coffee table were two (2) lighter fluid cylinders and a MAPP gas
cylinder with the torch tip connected, located at the base of the coffee table. Also, near the coffee table
were kitchen cleaners, a couple of bottles of window cleaner, and laundry on the floor fronting the coffee
table.

Mr. Rockett related that Mr. Miller used the torch (MAPP gas cylinder) for Ms. Gearharts jewelry making
and Mr. Millers hobbies that included: fixing cars, motorcycles, and for sweating plumbing pipe.

Mr. Rockett related that a lighter is used to ignite the MAPP gas.

Mr. Rockett related that hand tools were by the air compressor near the front door and a box of left over
motorcycle parts.

Mr. Rockett added that Ms. Gearhart buys stuff from Wish.com.

Mr. Rockett related that on Thursday (the day before the fire), a new air conditioner was purchased by Ms.
Gearhart and Mr. Mifier from Lowes. The new air conditioner was a 15000 Btu, 240v A/C unit that was
plugged into the outlet closest to the northeast corner of the living room. The air conditioner was off at the
time of the fire. The plug for the new air conditioner did not have to be changed, it was the correct plug
for the receptacle.

Mr. Rockett explained that the new air conditioner did not fit into the original air conditioner housing, so
the new air conditioner sat upon the old air conditioner placed on the floor. Mr. Rockett and Mr. Miller
were going to fix it on Saturday (July 15, 2017).

Mr. Rockett remembered that when he got downstairs, he thought the air conditioner caught on fire,
noting, Maybe it had an over draw on the 240 system or something.

Mr. Rockett related that the windows in the living room were open but the window blinds were closed.

Mr. Rockett did not think the washer or dryer were on at the time of the fire.

Mr. Rockett did not get up in the middle of the night.

Mr. Rockett had a cell phone that was charging. Its charger was plugged into an extension cord that was
connected to an outlet. The cell phone was a NextBook that Mr. Miller gave to him.

Mr. Rockett related that the phone was the size of a tablet computer. The phone was not holding a charge
so a replacement battery had been ordered by Ms. Gearhart.

There was an Apple computer on Ms. Gearharts computer desk and Mr. Rockett thought it was possible
the fire could have started there.

Mr. Rockett related that the middle bedroom door was partially open because the cat was in there.

Mr. Rockett related that the entry door to unit #2602 opened inward toward the kitchen.

Mr. Rockett related there was no fire in the middle bedroom or master bedroom, nor was there fire in the
bathrooms or kitchen.

Mr. Rockett related that the fire could have started from the NextBook cell phone, since it may have been
hot, but he did not know for sure.

Mr. Rockett related that there were no use of candles that he knew of, no mosquito punks, and no open
flames in unit #2602 at the time of the fire or prior to the fire.

I asked if Mr. Rockett received medical attention for his burns, and he corrected me stating, Theyre not
burns. However, he did confirm that he received medical attention from EMS personnel.

Mr. Rockett worked as an electricians helper and had done construction for nine (9) years so he knew the
difference between colors of the torch tanks, like blue for Propane cylinders and yellow for MAPP gas
cylinders.

Mr. Rockett related that the day before the fire, someone below unit #2602 was driffing into the concrete.
That was the only other thing he could think of that was out of place because the fire was shooting up
from the floor.Mr. Rockett allowed us to take pictures of injuries to his legs from crawling out of unit

The following was related to me by Mr. Thomas Miller.

Note: Attorney Scot Brower was present during the interview. It was unknown to me whether Mr. Brower
represented Mr. Miller.

Mr. Miller did not believe the fire started in his apartment.

In the early morning ofJuly 14, 2017, Mr. Miller was up late peeling the tint off his truck in the parking
garage of the Marco Polo building because he could not get a safety check.

Mr. Miller went to bed at approximately 0230-0300 hours, and ate a cup-o-noodles before going to sleep.
Mr. Miller woke up in the afternoon at around 1200 or 1300 hours, washed up in the bathroom, and went
outside to smoke a cigarette by his truck. He went outside to smoke because Ms. Gearhart did not allow
anyone to smoke in the apartment.

Mr. Millers morning routine was to get up, use the restroom, go downstairs and smoke a cigarette.

Mr. Miller left unit #2602 at approximately 1200 or 1300 hours, and did not go into the kitchen or into the
living room prior to leaving. He did not notice anything unusual in unit #2602 prior to exiting at that time.

Mr. Miller smoked his cigarette downstairs sometime between 1200 and 1300 hours, and ran the air
conditioning in his truck. While doing this, he decided to put gas in his truck because his gas was low.
After putting gas in his truck, he drove to get something to eat for Ms. Gearhart and Mr. Rockett.

While putting gas in his truck and getting food, Mr. Miller felt something was wrong, so he returned
home. Upon arriving home, he found fire trucks blocking the street and a fire at the Marco Polo building.

I asked Mr. Miller if he experienced electrical issues recently. Mr. Miller related that he sometimes got
shocked when turning the garbage disposal on or off.

Mr. Miller added that the receptacle near the front door had a broken ground plug stuck inside in it.

Mr. Miller related that a couple of months ago the buildings transformer went bad and was replaced.

Mr. Miller also related, The kid next door (unit #2601) might be growing something, buds or food. He
does not know maybe his light went bust and started the fire.

There was no recent work done on unit #2602, but a recently purchased air conditioner from Lowes was
installed in the living room on Thursday, July 13, 2017. It was a 15000 BTU/220v unit. The air conditioner
already had the 220 plug, so it was plug and play. Mr. Miller and Mr. Rockett hooked it up when they
got home because they were concerned that it did not work since it was accidently dropped on the way
home.

Mr. Miller stated, The air conditioner worked fme. It was blowing cold, no leaks, or sparks, no nothing,
and was turned off by Mr. Rockett. Mr. Miller related that Mr. Rockett did not like sleeping with the air
conditioner on and left the window open at night. Testing of the new air conditioner was done before Mr.
Miller went outside to peel the tint off his truck.

Mr. Miller related that the old air conditioner had been taken out of its housing and put on the floor. The
new air conditioner was placed on top of the old air conditioner but did not fit in the old air conditioners
housing. The new air conditioner was left outside the housing until the housing could be fixed. The old air
conditioner was unplugged.

Mr. Rockett had been living with Mr. Miller and Ms. Gearhart for about two weeks prior to the fire. Mr.
Rockett was a childhood friend of Mr. Miller and was sleeping in Mr. Millers truck in the garage until he
was asked to move in with them.

When asked what was in the living room, Mr. Miller related that they had recently wall-mounted a
television on the east wall.

Fronting the air conditioner, was Ms. Gearharts desk with an iMac atop the desk.

In the middle of the room was a coffee table with drawers. Inside the coffee table were: hand tools, a
lighter for Ms. Gearharts candles when she does yoga, assorted aerosol cans, and two butane lighter fluid
containers. One container was almost full and the other was 100% full.

There were no gasoline containers or gasoline operated equipment in unit #2602.

Mr. Rockett related to Mr. Miller that he woke up to the sound of an alarm and that the room was a smoky
bar-type smoke. Mr. Rockett also related to Mr. Mifier that he saw flames shoot to the ceiling and that he
called out for Mr. Miller at the bedroom door but Mr. Miller did not answer. Mr. Miller added that Mr.
Rockett said he kicked the door in and found Ms. Gearhart sleeping. Mr. Miller related that Mr.
Rockett puts the fire near the middle of the room away from the mauka side window.

There was a cubed booksheff, approximately 1 x 1 x 1 x 3 foot tall, made of particle board near the
northwest corner of the living room. On the book shelf was a candle that was not lit.

Mr. Miller related that every time Ms. Gearhart had used candles, He puts them out because he does not
want to die burning.

Mr. Miller related that the sectional couch in the living room was bought used on craigslist. The couch
was made of material similar to micro fiber.

of the occupants of unit #2602 parking stall and that Mr. Miller is a pro em ma

Ms. Lee related that Ms. Kim came home around 1000 hours on the morning of the fire to find the parking
stall belonging to the occupants of unit #2602 to be so clean. Diana sent a picture to Ms. Lee of the
clean parking stall.

Ms. Lee parks in a parking stall near to the occupant of unit #2602 and also related similar observations as
Ms. Kim of the parking stall belonging to the occupants of unit #2602.

On the day of the fire, at approximately 1320-1330 hours, Ms. Kim saw the girlfriend and another guy,,
talldng and talking.

Ms. Lee recalled that on Wednesday, July 12, 2017, Ms. Lee observed six coins placed on the trunk lid of
the occupants black vehicle. Then on July 13, 2017, she observed 5 coins placed on the trunk lid of the
same vehicle. She did not know the relevance behind these observation other then it seemed out of place.

Ms. Lee related that residents had taken pictures and complained to her about the occupants of unit #2602
since she was a board member.

The following was related to me by Mr. Leonard Wiffiam Rosa.

At approximately 1400 hours on July 14, 2017, two unknown males were leaving the Marco Polo from the
lobby to a waiting vehicle just outside the lobby entrance. Moments later, one of the guys started
knocking on the glass window to get Mr. Rosas attention. He told Mr. Rosa that there was a fire upstairs
overlooking Kapiolani Blvd. Mr. Rosa went outside to look and saw flames coming out of an apartment
above the 20th floor. Mr. Rosa then returned to the front desk to call 911.
July 14, 2017, was Mr. Rosas third day on the job.
Mr. Rosa did not know if the unknown males alerting him to the fire were residents or guests of the Marco
Polo.
Mr. Rosa described one male to have dark scraggly hair and beard and stated that he may have gotten into
a Toyota.
Mr. Rosa then went to the elevator lobby where he saw a lot of smoke coming from the elevator shafts.
Mr. Rosa thought the elevators were in manual mode.
Mr. Rosa went to the court yard area and looked up and saw smoke coming out from about the 20th floor,
on the makai/ewa side of the building. By the time Mr. Rosa got back to the front desk, the fire alarms
had been activated. According to Mr. Rosas recollection, this occurred approximately within a two (2) to
thxee (3) minute period.
Mr. Rosa remained at the front desk and did not know if the elevator doors opened manually but he was
able to monitor the elevators via the security cameras from the security desk located at the Marco Polo

entrance. Since this was his third day on the job, Mr. Rosa wasnt up to speed on emergency
procedures.
Both Mr. Rosa and Head Security Officer Sloan Namba were made aware of the fire about the same time.
Mr. Rosa added that The Marco Polo staff went into emergency mode.
Mr. Rosa worked the phone lines at the Marco Polo until leaving at 2130 hours.
Mr. Rosa described the following incidents, which were related to him by other security personnel:
-One day, Mr. Miller had been caught on video hanging on the side of a truck while it was being
driven by an unknown person, and security wanted to cite Mr. Miller for misbehavior in the parking
garage.
-Mr. Miller always had friends parked in the loading zone over the allowable fifteen (15) minute
period.
-Marco Polo security has had numerous problems with the occupants of unit #2602.
-Occupants of unit #2602 and their friends were unruly. Mr. Miller was rowdy and unruly as well.
-Mr. Miller and his friend frequently worked on cars in the parking space assigned to the occupants
of unit #2602.
-Occupants of unit #2602 were a pain in the butt for residents.
-It was rumored that Mr. Miller used and dealt drugs. It was also rumored he was operating a meth
lab.

The following was related to me by Ms. Keiko Izawa.

This hone call was placed by ATF Agent Ty Torco, and the conversation was translated by Kaye Izawa
who was Keiko Izawas sister.

On July 14, 2017, Ms. Izawa was getting ready to leave her apartment for work around 1415 hours. She was
scheduled to start work at 1430 hours and her workplace was near Ala Moana, which would take her 15-20
minutes to travel to. As Ms. Izawa left at about 1415 hours, she opened her apartment door and smelled
smoke in the hallway. The hallway was filled with black smoke and there was zero visibility. Ms. Izawas
door opened inward and she was able to open it enough to open the outer louvered door which was closed.

Ms. Izawa realized there was a fire somewhere on the 26th floor and quickly closed the door. Ms. Izawa did
not hear anything unusual prior to the fire, but after she opened her apartment door at 1415 hours, she
heard voices from the left side of her unit as she faced the hallway. Ms. Izawa related that one of the
voices belonged to a male.

Ms. Izawa stated that the smoke alarm on the 26~ floor had not sounded by the time she opened her front
door. Ms. Izawa went to her lanai. Then she tried to go back to the front door but could not see it since the
smoke started to fill her unit. She retreated back to the lanai closing all the windows in the bedroom on the
way. After reaching the lanai, Ms. Izawa closed the sliding door behind her to prevent smoke from coming
out to the lanai.

Ms. Izawa called 911 but did not get through. She did not remember how long it took for her to get
rescued.

Ms. Izawa related that as soon as she went to the lanai, she saw Ms. Kuwata in unit #2615 close to the
window. Ms. Izawa was afraid that Ms. Kuwata was going to fall out of the window. Ms. Izawa related that
it looked like Ms. Kuwata had a towel over her head and that it appeared her arms were covered with soot.
Ms. Izawa asked Ms. Kuwata if she was ok, and Ms. Kuwata replied yes. Later, Ms. Izawa didnt see
Ms. Kuwata.

Ms. Izawa related that she never saw the female occupant in unit #2613 (Ms. Dilley). Ms. Izawa saw and
heard the male occupant of unit #2613 (Mr. Reller) yeffing out of his bedroom window. Ms. Izawa
observed a lot of smoke coming out of the apartment #2613 from the lanai and the bedrooms.

Ms. Izawa never saw the mom (Ms. Dilley) the whole time.

Ms. Izawa related that Mr. Reller was in his bedroom near the window and was yeffing for his mother and
dog. Ms. Izawa recalled him yeffing mother.

Ms. Izawa initially asked Mr. Reller if the fire was coming from his unit, but then Ms. Izawa moved away
from his unit. Ms. Izawa did not hear a definite no to her question, but heard Mr. Reller yeffing for his
mother.

Shortly after, Ms. Izawa could see flames coming from the lanai area and middle bedroom of unit #2613
followed by the curtain rods and screen doors faffing. After that, she described hearing what she described
as an explosion. Then the yeffing stopped.

Ms. Izawa recalled that the occupants of unit #2613 would typically leave the entry door and lanai door
open for ventilation and airflow. Ms. Izawa also added that when she would leave for work, she could
typically see into unit #2613.

Ms. Desha recalled that Britt Reller (deceased victim in unit #2613) and Ms. Desha were good friends. On
July 14, 2017, at approximately 1408 and 1410 hours, Mr. Reller called Ms. Desha with the companys duty
phone to tell her there was a fire.

Ms. Desha related that Mr. Reller told her that he was taking a nap in his bedroom when he woke up with
his room fully filled with smoke.

I asked Ms. Desha if Mr. Reller mentioned anything about seeing a fire. Ms. Desha related that he kept
repeating There is so much smoke I cant see. He was asking her where is the fire coming from? He
kept saying, There is so much smoke.

Ms. Desha related to me that in her conversation with Mr. Reller, it appeared he was unable to leave his
room. While Mr. Reller and Ms. Desha were on the phone, she could hear him call out for his mom and
dog with no answer from his mother. Mr. Reller repeatedly said, There is so much smoke.

Ms. Desha related to me that she didnt hear any alarms going off in the background of their phone call
and really did not know what was going on.

Mr. Reller did not know where the fire was. He asked her several times, Where is the fire?

Dale and Jere Gearhart are Laura Gearharts parents.

Dale Gearhart related to me that Laura Gearharts sister Lisa Gearhart passed away last year, November 5,
2016, and it had been hard on Laura and her family. Laura and Lisa were best friends and extremely close.

Dale Gearhart added that Lisa and Laura lived together at the Marco Polo in unit #2602 prior to Lisas
death.

Dale Gearhart related to me that Laura lost a cat and three birds in the fire and that the cat was Lisas.
Losing Lisas cat made the incident much more difficult for Laura.

Dale Gearhart related that Laura stopped drinking wine and alcohol after Lisas death.

Dale Gearhart met Lauras fiance, Thomas Miller, when Mr. Miller and Laura visited Dale and Jere
Gearhart in October 2016. Dale Gearharts impression of Mr. Miller was that he was quiet. Dale Gearhart
added that all he hoped for Laura was that she found someone who treated her with care, love, and
support. The main thing was that Laura was happy.

Dale Gearhart related that as far as he knew, Laura did not smoke.

Mr. Tagoreerwin stated that when the fire occurred on July 14, 2017, Mr. Tagoreerwin was not home, and
his friend called him to tell him that his apartment was on fire. I informed him that the fire appeared to
have started in his neighbors apartment, and not his.

Mr. Tagoreerwin related to me that he woke up on the morning of July 14, 2017, at around 0900 hours,

The following was related to me by Mr. Sloan Namba.

Mr. Namba related to me that he received a radio call from the front desk regarding an apparent fire on
the upper floor of the Marco Polo. He was on the 4th floor when he received the call and ran downstairs to
the fire annunciator panel on the ground floor/#2 stairwell (middle stairwell). No fire indication was
noted at the time. Thi~ panel could be activated by pulling the T-bar of any fire pull station.

Mr. Namba then went to the fire alarm station in the number 3 electrical room (lobby area) where he
discovered the smoke detector was activated on the 26th floor.

Mr. Namba then grabbed a bull horn from the security front desk, ran to the back courtyard of the Marco
Polo, and started an emergency announcement. He did this because there was no public announcement
system.

Mr. Namba observed debris and spot fires on the grass fronting the Marco Polo.

Mr. Namba related to me that he thought the elevators were recalled at the time the smoke detector went
off on the 26th floor. However, they were not working so he went to the elevator call box. There, he

observed the barrel key switch spinning freely, so he pulled the bypass panel off at the #1 elevator entry
door and manually connected the switch to operate the elevators.

Mr. Namba related that sometimes Marco Polo security would have to bypass the system when there was
a false alarm and the elevators would get recalled. The bypass purpose was to override the system and
put it back to service, Mr. Namba explained.

Around 2:15 p.rrt, I was on the 4~ floor with RS Sloan where I was showing him a house rule
violation by unit #0415. Around 2:18 p-rn-, I heard the word, FIRE, over the radio. Sloan took
off runnIng toward the middle stairwell. I went to the Ewa stairwell. I heard the fire alarm ring
while running down the stairs. I caine out of the Ewa stairwell and ran around to Kapiolani
Boulevard and looked up. I radioed to front desk to call 9-1-1 for fire. Then I went to the front
desk. At 2:19 p.m., I arrived at the front desk and asked Alliance Personnel, Leonard, for the
bullhorn. RS Lead Sloan came up and retrieved the bullhorn and went to alert residents from
Makai side of building. I called 9-1-1. The dispatcher told me that they already have the fire
department on the way to Marco Polo.

Next~ I assisted in preparing to receive fire trucks. I made sure sliding doors were open. RS
Sloan and I went to the elevator lobby to prepare elevator for fire truck. I went back to the
front desk and grabbed our ADA list I gave a copy of the list to a fire fighter and pointed out
units on 26th floor: 2605 and 2610. Then P went outside and assisted residents coming out of
the building. I gave another firefighter another copy of the same ADA list. I went to the
elevator lobby and saw that the firemen were having a hard tine with the phase 2 of the
passenger elevators. So I radioed RS Sloan for assistance. I eventually radioed staff for the
freight elevator because firemen seemed to have a hard time in elevator lobby. Around 2:28
p.m., Sloan and I ran around to the rear loading zone for the freight elevator. Victor assisted
with the freight elevator. He brought it to first floor and explained to firefighters. I radioed to
the front desk to call Otis Elevator Company. I went back to the front of the building.

A young lady asked about an elderly female relative. I went into the middle stairwell (stairwell
No.2) usIng myX-key. Iran upstairs and saw a brown.sklnned elderly lady with a cane similar
in skin complexion to the younger female at the circle drive. I assisted the elderly lady down
the rest of the stairs and connected her with her younger female relative at the circle driveway.
I went back into the No. 2 stairwell and assisted others I escorted
Joani Taylor and Bill Faulkner, down some stairs in middle stairwell until Bill felt he could not go
any further. I asked them to wait at the vent holes near the middle stairwell. I ran into HPD
officers who were knocking on doors. I helped an HPD officer by pushing wheelchair passenger,
Charlene Tsuji, from Stairwell No. 3 (Diamondhead stairwell) to Stairwell No. 1
(Ewa stairwell) to wait for the freight elevator. I called it out over the radio. HPD officer
remained with Charlene. I went back to the No. 2 stairwell and could not find unIt #2 104
residents, I called out to them to no answer, While on the 21~ floor, HPD knocked on
and Joani Taylor answered the door. I told them they can go to the freight
elevator for pick up. I eventually heard from either HPD officers or firefighters that the freight
elevator stopped working. RS Sloan attempted to get one of the building passenger elevators
operating. At 3:29 p.m., I made it to the freight elevator opened on the 24th floor. i radioed
the staff. There was plenty of water. I attempted to get the freight elevator moving but failed.

RS Sloan radioed that elevator U4 was running. I left the ~4th floor Lwa stairwell landing
around 3:42 p.m.

Two females in a unit asked me if they should leave their unit. I went and asked firefighters at
the Diamondhead stairwell. They asked the location of the ladies. I told them. HFD firefighters
told me to tell thorn to shelter in place. I went back and told them

At 3:48 p.m.. I saw apartment #2511 residents (couple) be escorted on Diamondhead stairwell
from 261h floor down to 24th floor by an HFD officer. I went to elevator #4 to make sure it did
not leave without the couple. Elevator tf4 would no longer run. It just remained open.
asked firefighters about going to the elevator machine room. One firefighter told me that his
captain would have to authorize it. I saw and talked to an older Asian couple at a unit. They
put me on a cellular phone with their son who spoke better English. asked firefighters about
the couple and was told to tell them to shelter in place, also. I told them through their son on
the cellular phone. Eventually, I talked to a fire captain and explained that I could reset
elevators from the elevator machine room. He authorized a firefighter and me to go to the
elevator machine room on the roof and try to reset elevators to get and elevator running again.
HFD officer, J. Yosbimura, and I went up the middle stairwell to the roof. He and I went to the
elevator machine room. I shut of all four passenger elevators around 4:01 p.m. Then I turned
them all back on. None of them operated normally. I shut them off then turned them on again.
Still, none of them operated normally. Yoshimura and I headed to the freight elevator n,achine
room but the path to the room took on a lot of 5moke. So I showed J. Yoshimura, my key
marked, X Key, to open the door to the freight elevator machine room. I explained what to turn
off and back on. i. Voshinura went through the smoke to the freight elevator machine room.
He returned the key to me, afterwards.

HFD officer, i. Yoshlmura, and I checked floor as we came down. We ran into a Caucasian male
resident on the 32nd floor. His name is Ronald Chiarttino I
explained to Ronald that he needs to evacuate. Around 4:30 p.m., It was announced over the
radio that Otis Elevator Company was on site. I radioed that we have three of us on 32fltt floor.
Ronald, Yoshimura and I went to the middle stairwell and waited by the vent holes. Smoke
started coming into the middle stairwell. At 4:34 p.m., Otis Elevator Company tech, got
elevator 1*3 running. RS Sloan announced it over the radio. I radioed again that we have three
of us on the 32nd floor. The resident, Yoshimura and I went to the OH stairwell. We saw a cloud
of smoke rising from below. So we went back into the 32Ad floor hallway. HPD officer
Yoshimura lent us air from his uniform. At 4:44 p.m., elevator #3 opened up on the 32w floor
Yoshimura, Ronald and I joined Otis Elevator Company tech, Calvin, and another firefighter
inside elevator #3. We rode down to the lobby. I retrieved the Mens restroom key for a
firefighter at the front desk and showed the location of the restroom. Alliance Personnel
Leonard gave me apartment numbers of persons who still needed help to evacuate. I wrote
down and gave apartment numbers from call ins to the front desk and employees over the
radio to firefighters at the elevator lobby After a while, I started writing the apartment

numbers down and just telling the HFD firefighter at the elevator lobby that relayed to I-tFD
personnel

Around S p.m., we all learned that elevator $43 became stuck open on the 22~~ floor. Two
elevator technicians, Calvin and Lincoln, tried to get an elevator or elevators going from the
elevator lobby. They were unsuccessful, I escorted both technicians, Calvin and Lincoln, to the
garage elevator. We rode ii. up to the 6th floor. I led them over the 6th floor crossover and
walked up the Diamondhead stairwell (No. 3). We passed by residents, Phil Rosenberg and
Karen Hastings, in the stairwell around the 181h floor. I told them they should get on down the
stairs because the fire continues, The two technicians and I made it up to elevator fl3 on the
22nd floor by 5:12 p .m. [he technicians tried but could rot get it going from the 22nd floor. The

technicians and I discussed the situation. One of the two technicians asked me if I had a key to
the elevator machine room on the roof. I told them, yes. I lead technician, Calvin, through
middle stairwell (No. 2)to the roof then to the elevator machine room. I opened the door for
him. Calvin shut off elevator #3 at 5:40 p.m. while the other technician was working outside
the elevator car. Both technicians communicated with each other via cellular phones. At 6:04
p.m., Otis Elevator technician, Lincoln, closed doors of elevator #3. Eventually. Lincoln made his
way on top of elevator car #3. Around 6:30 p.m., Calvin and I left the elevator machine room
and went down to the 24th floor where HFD had set up staging. EMS was there also. Lincoln
opened elevator #3 to the 24Ih floor while riding on top of the car. Firefighters loaded the
elevator with equipment. Calvin rode down with them to the lobby.

Around 7:03 p,nt, I got into the elevator with four elevator technicians including Calvin and our
regular mechanic, Owane. We rode up to the 35Ih floor (penthouse) Then I rode elevator back
down to the lobby. Eventually, I received a phone call from Owane wherein he told me he did
not have a key to the elevator machine room. So I rode elevator #3 up to the 35Ch floor
(penthouse) then walked up middle stairwell to the elevator machine room where I opened the
door for elevator technicians to gain access. They worked to get at least one mcne elevator
running. I returned tothez4th floor.

Around 7:58 p.m.. I got into elevator car #3 and rode it down to the lobby wrth firefighters and
elevator technician, Lincoln. I went to the front lobby and checked on operations. Alliance
Personnel, Leonard, RS Lead, Sloan, and HPD were at the front desk. RS Debra came to front
desk. I ran into fire inspector, Michael Carter, at the elevator lobby around 8:26 p.m. I
answered inquiries frorri him.
