II. Date of Psychological Test

Ms Cabal took the one session of the tests last September 11, 2017. The tests
administered are Mental and Psychological Test.

III. Personal History

Ms Cabal is born via caesarean section, no known complications and abortion

attempts of the mother. Her parents are married when she was born.

She spent most of her childhood inside their house. She seldom went outside to
play with the other kids because her mother sometimes restricted her to do so.

At the age of 5, she began her preschool years. She excelled in academics and
received a lot of medals, ribbons, and always tops the honorary ranking. She gained a
lot of friends and most loved by the teachers. These motivated her to study well that is
why she comes home from school; she read and studies her lessons with the help of his
father. She continued being an achiever until high school.

Although Ms Cabal is surrounded with friends, she still has poor interpersonal skills
and has a little problem with maintaining a good bond with other people.
 The most traumatic experience she had was when her mother would point a
knife and threaten her life every time she gets mad at her. She claimed that she doesn't
have a good relationship with her for her mother usually only see the things she'd done
wrong and would hit her as a punishment. She developed fear and anger towards her
mother. On the other hand, she thought that she's useless and self-degrade.

Two years ago, her father died because of cardiac arrest. She was very close to
her father for her father always defending her against her mother. When her father
died, Ms Cabal felt she lost not just a father, but a best friend as well. Since then, she
was depressed but would act as if everything is okay.

Last year, she found out that her mother was living together with a different man
and the two were planning to get married someday. Ms Cabal felt that her mother
already abandoned her because her mother seldom stays at their house. This caused
her anger to escalate.

She is currently living with her mom's cousin at Quezon City and studying BS
Psychology at Polytechnic University of the Philippines.

IV. Observation

A. Before the test

The client was observed to be prepped before taking the exam. She sat properly
and readied her pencil and eraser. She looked a little bit anxious but she eagerly
listened to the given instructions.

B. During examination

The client was observed to be focused on reading and answering the questions.

C. After examination

The client was observed to be relieved but still a little bit of anxious at the same
time. She usually smiles upon realizing the mistakes in her answers.

Interpretation

Figure 1 shows the mental ability of Ms Cabal on different subtests.

The table showed that her highest raw score (RS) is Reading Comprehension with
26 out of 60. While her lowest raw score (RS) is Verbal Reasoning and Math Application
with 10 out of 25. These suggest that she has poor arithmetic abilities while she has a
good apprehension. It could also mean that numbers are giving her a hard time while
she easily understands the context of literature.
 The table also showed that her general evaluation (GE) is Average. This implies
that her cognitive capacity and performance is typical and usual and not beyond the
normal or below IQ. She may effortlessly understand the fundamentals of certain
content but she could also have a difficulty deriving sometimes.

Figure 2 shows the emotion percentile of Ms Cabal based upon the adult norms
for females.

She is extremely depressed and feels sad and gloomy. She doesn't much enjoy
life. She has a subtle introvert personality. Her Aggressive score is also high with a score
of 97. She probably tends to be blunt, quarrelsome and be annoyed easily.

Her high score on Gregarious implies that despite of being introvert, she
sometimes tried to interact with different people and create new bonds with them. But
her low score on Trustful means that she doesn't easily give her trust to the people
around her. She isn't gullible and does not take things at face value.

Her average score on Distrustful suggests that she tends to be passive-aggressive

and hostile towards other people. The low score on Timid implies that she is less cautious
and likes to take risks despite of getting into trouble.

The score on Dyscontrolled implies that she can be impulsive and adventurous
sometimes and like to try new things. On the other hand, her zero percentiles on
Controlled suggests that she tends to live her life on a day-to-day basis and she has low
self-control.

With a high score of Gregarious and a low score of Timid, she has the
combination of High Gregarious + Low Timid personality combination. This indicates that
Ms Cabal likes to be with people but has lack of conscience and not too responsible.

VI. Conclusion

Based on the tables and derived interpretations from it, Ms Cabal has an
average mental ability and is depressed and aggressive.
 Her average intelligence quotient (IQ) implies that she has an average reasoning
and problem-solving skills. She has a good comprehension and good arithmetic solving
skills.

Her normal scores on her subtests suggest that she can comprehend easily but
sometimes she can't.

Her high percentiles in Depression and Aggressive could have been the effect of
her poor relationship with her mother, her traumatic experiences with her mom when
she was still a child and as well as the death of his father two years ago.

These factors led her to become sad, gloomy and depressed because she was
very close to her father and she felt like she lost a huge part of her life. It also led her to
build anger and hostility towards her mom because of her mother's maltreatment to her
during childhood.

VII. Recommendation

For her mental ability, I recommend Ms Cabal to exert effort in learning

mathematics and study more. For her intrapersonal aspect, I recommend her for
counseling so she could release all the heavy feelings inside her chest and to be more
socially interactive as well.
