Introduction to Gymnastics
For boys and girls ages 4-5 and in Kindergarten for Winter 2018
The Recreation Department is pleased to provide our 43rd year of quality recreational gymnastics instruction. All participants
must be registered members of the Recreation Dept. $24 residents $26 non-residents for fiscal year June 1, 2017 May 31, 2018.
Registration: Nov. 6 for residents and non-resident returning students. Nov. 13 for new non-residents.

An introduction to gymnastics apparatus, tumbling and

movement education for boys and girls age 4-5-6 in Pre-K Pre- K children (age 4-5)
Child must be 4 years old before the first day of class!
and Kindergarten. The children will learn about and use
the floor exercise, balance beam, uneven bars, vaulting, MONDAY 1:15-2:00pm
trampoline, tumble track, and rope. Children who are 3 TUESDAY 2:00-2:45pm
years old should register for Tumble-Time 3, the curriculum WEDNESDAY 1:30-2:15 pm
for which is a wonderful lead-up to Intro. to Gymnastics. THURSDAY 1:00-1:45pm
Proof of age may be asked of you at registration. THURSDAY 1:45-2:30pm
Children must wear athletic clothing no skirts or jeans FRIDAY 12:30-1:15pm
no jewelry (including earrings), hair must be tied back. FRIDAY 1:15-2:00pm
SATURDAY 9:00-9:45 am
NOTE: Class activities start promptly. Children must arrive on
time. Children who arrive 10 minutes late or later will not be
admitted. Make-ups for missed classes are limited. Kindergarten
for 2017-18 school-year
NO REFUNDS Credit may be issued for withdrawal before

COMPLETE THIS FORM AND SIGN:

This programs goal is to provide gymnastics instruction to children in a small group setting. There may be potentially 6 groups of (9) children
actively involved in gymnastics, simultaneously performing on the various gymnastics apparatus. All participants must be able to follow safety
guidelines as presented verbally by the instructor and visually posted near the equipment. All participants must also be able to follow direc-
tions of the instructor, remain with his/her group at all times and respect the other children in the group. If you suspect that this may be a chal-
lenge for your child, please discuss this programs suitability to your childs needs with the program directors before registering. All discus-
sions and information provided by parent/guardian are confidential.

Note: If gymnast sustains an injury or contagious illness or condition that requires visit to physi-
cian, a physician notice of clearance to resume participation in gymnastics must be submitted to
Rec. office.

I certify that I have read and understand the above notification and that my child is in good health and capable of participating in gymnastics group instruction. The Village of Rockville
Centres Recreation Dept. recognizes its obligation to make our students and their parents aware of the risks and hazards associated with the sport of gymnastics. Students may
suffer injuries, possibly minor, serious or catastrophic in nature. By its very nature, gymnastics carries a risk of physical injury and this risk cannot be eliminated. Parents should
encourage their children to follow all the safety rules and the coaches instructions. I give permission for the Recreation Dept., its coaches and other staff members to take whatever
emergency measures are judged necessary for the care and protection of my child while under its supervision.. In case of a medical emergency I understand that my child may be
treated by the local emergency resources (police, rescue squad) if deemed necessary. I waive and release all rights and claims for damages that my child or I may have against the
Village of RVC or its representatives. With all the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child participate in the pro-
gram offered by the Recreation Dept. I am signing this waiver of liability, consenting and releasing the Village from all liability, claims and causes of actions arising out of activities at
the Parks & Rec. Dept.
