Termination of Employment

We regret to inform you that, your employment with has been


officially terminated with effect from 18.05.2016 as per the disciplinary inquiry held on
18.05.2016. It was evident (and as admitted by you) that on 8th May 2015 during
working hours (while engaged in services) you have been in unsound mind after
consuming alcohol at the work premises thereby creating an unpleasant environment
causing severe difficulties to the organization, its employees and customers. Your act
has been of bad precedence to the organizations goodwill and the staff engaged at
.. Hence due to this reason we are compelled to terminate your
services with immediate effect.

Furthermore, a similar warning have been issued to you on many instances as listed
below, however the management regrets to inform that you have not improved your
performance.

1st warning issued on 14.03.2016 as a no of void receipts totaling to a very large


amount was raised from October 2015 to March 2016 with unacceptable reasons.

On 22.04.2016 food was given to a client without taking the payment and at the
end of the day the bill was settled using the float. Even after
warning the same act was repeated on 25.04.2016.

On 08.05.2016 been drunk during working hours and a client complained of the
same.

From 14.05.2016 & 15.05.2016 not been punctual and informing late that that you
are unable to make it to the shift.

Furthermore, as per your letter of employment (signed upon hiring) at


., the organization has found that you have breached the
following clauses stated under Disciplinary Action, which warrants termination of
service.
15.2 Acts of insubordination or dishonesty.
15.3 Drunken or disorderly behavior
15.5 Absences from work without approved leave entitlement
15.6 Habitual late attendance
15.7 Breach of any term or condition of employment.
15.8 Neglect of work or duty.
15.9 Any act of omission, which will result in loss or disrepute to the company.
15.11 Any act or omission, which is prejudicial or detrimental to our Interest
15.12 Loss of confidant

We the management consider this as a serious breach of conduct and we hereby wish to
state that the above act of misconduct is not acceptable by any means of the
organizations standards, and that you have violated organizations policies. Your act has
severely affected the managements confidence and trust placed you in the capacity of a
. Hence your services have been terminated with immediate effect from
