RISK RATING METHOD

Once all the hazards have been identified and the control measures currently in place have been highlighted, the risk
rating can be completed. Trivial risks should be included but can usually be ignored.

Risk Rating -Two things contribute to the risk arising from a hazard

1. How likely it is that the hazard will cause an accident (Likelihood)

Unlikely If other factors were present an accident/illness/incident might occur but the probability
1 is low

Possible An accident/illness/incident may happen if additional factors precipitate, but unlikely to

2 occur
3 Probable An accident/illness/incident will happen if additional factors precipitate
4 Very Likely An accident/illness/incident will happen if additional factors are not eliminated
5 Certain If work continues as it is there is almost a 100% chance that an accident will happen

2. If it did cause an accident how severe the consequence would be (Severity)

1 Minor Minor injury/ First Aid/lost time/delay

2 Low Over 3 day injury
3 Medium Reportable injury/ disease/ occurrence. Temporary disability
4 High Permanent total/partial disability
5 Major Multiple/single death

RISK MATRIX

Likelihood 1 2 3 4 5
Unlikely Possibl Probabl Likely Certain
e e
1 Minor Low Low Low Low Medium
2 Low Low Low Medium Medium Medium
3 Medium Low Medium Medium High High
4 High Low Medium High High High
5 Major Medium Medium High High High
Severit

The risk rating for individual hazards can then be classified as follows:
The higher the score, the higher the risk and the higher the hazard priority for control measures

Individual Risk Rating

This is the usual rating you would expect and in the majority of cases is the normal
1-4 LOW condition using the correct control measures, but an accident or incident could still
happen.
Record the finding of your assessment and introduce additional control measure to
5 - 10 MEDIUM improve the Risk rating. Certain activities will be out of your control so every effort
should be exercised when carrying out such activities with medium risk
11 - 25 HIGH Stop immediately/do not start activity and seek further advice.

body work , Heat The base must be firm and level enough to support the load of scaffolding, workers,
extreme of stress, and materials. Sills and base plates are required on any soil or unstable ground
heat Fatalities condition, or where any leveling adjustment is needed. The sill must be sound, rigid,
and capable of supporting the maximum loads without settlement or deformation.
Adjustable base plates must be adjusted within the limits specified by the
manufacturer or supplier.
The frames must be plumb and level and spaced to adequately support the loads.
If uplift could cause the components to separate, locking pins must be used and the
components secured. Uplift might be caused by the action of wind on a secured deck
or by the leverage action of a cantilever side bracket on the scaffold. All joints must
be pinned on rolling scaffolds and free-standing towers.
With a rolling scaffold or free-standing tower, the platform height must not exceed
three times its smallest base dimension (3 to 1 rule).
A scaffold built above the 3 to 1 rule must be effectively guyed or secured to a
building or structure to prevent overturning. The guying of a scaffold may require the
direction of a professional engineer. Other methods of tying the scaffold should be of
a push-and-pull type mechanism and located no more than 6 m (20 ft.) vertically and
6.4 m (21 ft.) horizontally, after the first tie has been installed to comply with the 3 to
1 rule.
Guardrails must be installed on all open sides of the platform where a person could
fall a distance of 3 m (10 ft.) or more. The top rail must be placed 102-112 cm (40-44
in.) above the work surface. An intermediate rail must be placed halfway between the
top rail and the toe board, if one is provided, or halfway between the top rail and the
work surface if no toe board is provided. The guardrail must be able to withstand a
load of 550 N (125 lb.) applied at any point on the top rail.
Toe boards must be installed on all the open sides of a platform where it is possible
for tools and or materials to roll off. The top of the toe board must be at least 10 cm
(4 in.) above the platform. If loose materials are to be stacked above the height of the
toe board, then the height of the toe board must be increased or mesh panels must
be installed to prevent materials from falling off the scaffolding.
Ladder access may be vertical or sloping. Ladders must be secured at the point of
support and must extend approximately 1 m (3 ft.) above the landing. A vertical
ladder must have a clear space of at least 15 cm (6 in.) behind each rung
Ensure all materials used are free of defects (cracks, welded or deformed tubes)
Ensure all scaffolding are green tagged prior to use and unsafe, under construction
scaffolding must be red tagged.
Ensure weekly inspection is carried out and the tag renewed.
Ensure the area of scaffolding erection is barricaded and with warnings tapes & signs
