Sir,

I, Madhavi Khanna, am the Informant undersigned. I am working at a

private company [name and address] and reside at Hauz Khas, New Delhi.
[Comment: Note how the address of the place of work of the complainant and
not residential address is mentioned. Sometimes, the address given to the
police may leak into the wrong hands]

On the night of 16.5.2015, I was surprised as I got a call at around 2 am, I

received a call on my cell phone having number [insert], from number
[insert]. I only heard heavy breathing on the other side but nobody spoke. I
thought it was a bad connection and hung up the phone. At around 2.15 am
on the same night, the same number called me again on my mobile number
mentioned above. This time, a male caller spoke, sounding drunk and slurring
in speech and told me in Hindi stating that he wanted to meet me. I told him
sternly not to call again as he was harassing me. On 17.5.2015, at around 1
am at night again, the same person number called me again. I answered the
call and the person on the other line threatened me with dire consequences if
I did not meet him. He further said that he knows where I work and can find
me there and started abusing me. I cut the call at this stage and informed my
father.

I fear for my life and safety and am worried that some harm may come to me
as despite my warnings, the caller has continued to contact and threaten me.

I therefore request that an investigation be conducted into who this number

belongs to and an FIR be lodged against the caller under sections 354-D, 506,
509 of the Indian Penal Code as well as other offences which may be made
out. [Comment: Even if the informant does not know the exact sections, they
can still atleast narrate the fact situation in detail]

I would also request you also to provide me with police protection in the
meantime. [Comment: This is an additional relief which is sought]
