LIVING WILL OF CITHY D.

OBNAMIA

I, CITHY D. OBNAMIA, Filipino citizen, of legal age, single, born on the 9th of August,
1991, a resident of #1 Area I Republic Avenue, Barangay Holy Spirit, Quezon City, being of
sound and disposing mind, do hereby declare that this is my Living Will and that I voluntarily
make this declaration. I intend this declaration to be honored as the expression of my legal
right to authorize or refuse medical treatment.

My desires concerning medical treatment are:

If all following conditions are met,

1. If I become terminally ill or permanently unconscious as determined by my


primary doctor and at least one other doctor;
2. Where my death would occur without the use of artificial life sustaining
procedures;
3. If I am unable to participate in decisions regarding my medical care; and
4. Where I am unable to communicate my wishes.

I direct my healthcare providers to refrain from any treatment that involves


artificial nutrition or artificial hydration. However, I do want maximum pain relief even if this
might hasten my death.

I appoint MARIFE PAGLINGAYEN, of Novaliches, Quezon City, to be my health


care agent with full power of attorney to make health care decisions based on this document
and any views I may have previously expressed. In situations where my wishes are unknown,
I instruct my agent to make decisions based on what she considers to be in my best interest
and in keeping with my own personal values. The authority of my health care agent becomes
effective when my primary doctor determines that I am unable to make my own health care
decisions.

My family, the medical facility, and all doctors, nurses, and other medical
personnel involved in my care shall have no civil or criminal liability for following my wishes as
expressed in this declaration.
I may change my mind at any time by communicating in any manner that this
declaration does not reflect my wishes.

I sign this document this 27th day of October 2017 at #1 Area I, Republic
Avenue, Barangay Holy Spirit, Quezon City, after careful consideration and full understanding
of its meaning and consequences.

We sign below as witnesses. This declaration was signed in our presence. The
declarant appears to be of sound mind, and to be making her designation voluntarily, without
duress, fraud, or undue influence.

REFLECTIONS

Atty. Angelo M. Cabrera's Thy Will Be Done has practically recommended


reasonable options when the time of death or near death comes. Considering that accident,
illness, or death are inevitable, emotions emerge immediately from the family or friends of the
patient or deceased, and often emotions do not contribute to the best decision needed at that
certain time.

In my opinion, Atty. Cabrera's recommendation is enlightening which is


applicable to those who do not desire to leave burden on their loved ones due to their own
worst fate, especially to those parents to their children, bread- winners who are not parents,
or others who are similarly situated. I, myself is the one supporting the family given that my
eldest siblings sent me to college in order that I may be able to support the family in return. As
I chose this recommendation to be applied to me if the time comes, I believe that my personal
decision will be expressed and truly, my personal values will be reflected if followed.

Nowadays, Filipinos value education as one of the best mode to self-


improvement and incidental to it is the greater opportunity of financial stability. To protect
such goal in life, one must know about this making of Living Will.

CRITIQUE

Preparation is always a good investment for future events. However, no one can
determine all surrounding circumstances of that certain event. Still, it is advisable for a person
to be at least prepared of it.

In the concepts of the Thy Will Be Done book, it is based on legal grounds as
well as common experiences. But it does not encompass unique situation of every individual
and the very personal consideration by him thus, this will serve merely as a guide and may be
amendable as to his wishes. This is the issue that should be taken into account.

At any rate, Living Wills should be recommended and hopefully, the government
will take a positive act and create a system to introduce this to Filipinos to avoid or lessen
chaos and disputes after one has died.
