TRAINING CONTRACT

CHED FACULTY TRAINING FOR THE TEACHING OF THE NEW GENERAL

EDUCATION (GE) CORE COURSES: SECOND GENERATION TRAINING

The (Sending Higher Education Institution), a higher education institution

with principal office at
___________________________________________________ represented herein
by its head, ________________________ and hereinafter referred to as SHEI;

- and

(name of Faculty Participant) of legal age, Filipino and residing at

__________________________________ hereinafter called the GRANTEE.

In consideration of the actual financial support for the duration of the training
program which will be released to the GRANTEE, the SHEI shall allow the
GRANTEE to participate in the CHED FACULTY TRAINING FOR THE TEACHING
OF THE NEW GENERAL EDUCATION (GE) CORE COURSES: SECOND
GENERATION TRAINING at the (name of Delivering Higher Education
Institution) for a duration of __________commencing on _______________.The
parties, moreover agree that do hereby knowingly agree to the following terms and
conditions:

The SHEI shall

1. Affirm the eligibility of the faculty applicant based on the criteria approved
by CHED;
2. Provide official time to the GRANTEE during the course of the training;
3. Provide the GRANTEE, travel and accommodation allowance during the
course of the training;
4. Coordinate with CHED in monitoring the progress of the GRANTEE under
the program;
5. Terminate the training program of the GRANTEE due to:

4.1 the demonstrable inability of the GRANTEE to complete his/her

training course;
4.2 commission of any act of immorality, drunkenness, dishonesty,
discourtesy, and any form of misconduct; and
4.3 conviction of any crime by a court or a competent administrative
body;

6. Inform CHED of the termination of the training program of the GRANTEE

 caused the reasons stated above
7. Ensure that the GRANTEE render at least one term of return service to
the institution for every 3-unit course completed by teaching the GE
course in which the faculty was trained;
8. Reimburse CHED the training cost of the GRANTEE in case the SHEI
recalls the latter or the SHEI terminates the program of the GRANTEE for
the reasons stated above; and
9. Provide the CHED a certification that the GRANTEE rendered return
service to the SHEI relative to his/her attendance to the CHED Faculty
Training for the Teaching of the New General Education (GE) Core
Courses: Second Generation Training as required in the program.

The GRANTEE shall

1. Participate in the Faculty Training for the New General Education Core
Courses (Second Generation Training) program;
2. Reimburse CHED the total assistance released to him/her in case of
willful abandonment of the training program, gross misconduct and non-
compliance with the terms and conditions stipulated in the Training
Contract; and
3. Render at least one term of return service to the institution for every 3-unit
course completed by teaching the GE course in which the faculty was
trained.

WHEREFORE, both parties signify that the above terms and conditions have
been discussed to them and that they fully understand and agree to all the terms
thereof.
