GSIS POLICIES ON THE DESIGNATION, CHANGE OR REVOCATION THEIR

BENEFICIARY/IES UNDER LEP OR ELP:

Below are the policies on the designation, change or revocation their beneficiary/ies under LEP or ELP
effective 23 June 2016:

1. The beneficiaries of the policyholder shall automatically be his or her legitimate spouse
as well as his or her legitimate, legitimated, adopted and declared illegitimate children.

2. Designation of beneficiary/ies under the Enhanced Life Policy (ELP):

Who are allowed to designate beneficiary/ies:

a) Active member without legitimate spouse and legitimate, legitimated, adopted and
declared illegitimate children; or
b) Active member with legitimate spouse and legitimate, legitimated, adopted
and declared illegitimate children but one of the primary beneficiary/ies
died; or
c) Active member revoked legitimate spouse as beneficiary and is childless.
d) Active member is not yet 65 y.o. (i.e., 64 y.o. and below) at the time of request.
e) No filed application for compulsory retirement.
f) With submitted Designation or Change of Beneficiary/ies Form (signed by the AAO).

Who may be designated as beneficiary/ies?

3. Change of beneficiary/ies under the Life Endowment Policy (LEP):

Who are allowed to change beneficiary/ies:

a) Active member without legitimate spouse and legitimate, legitimated, adopted and
declared illegitimate children.
b) Active member is not yet 65 y.o. (i.e., 64 y.o. and below) at the time of request.
c) No filed application for compulsory retirement.
d) With submitted Designation or Change of Beneficiary/ies Form, signed by the AAO.

Who may be designated as beneficiary/ies?

4. Revocation of spouse-beneficiary under LEP or ELP:

Who may be allowed to revoke the spouse-beneficiary?

a) Active member who has sufficiently proven that he or she is separated-in-fact from his
or her legitimate spouse due to the fault of the latter.
b) Active member complied with the submission of the following documents:

Duly Notarized Affidavit of 2 Disinterested Persons or Witnesses stating that


the policyholder and his/her legitimate spouse are separated-in-fact (i.e., no
longer living together) and the underlying circumstances showing that such
separation was due to the legitimate spouses fault; or
Barangay Certificate issued under oath by the Punong Barangay of the
barangay where the policyholder is residing stating that the policyholder and
his/her legitimate spouse are separated-in-fact (i.e., no longer living together)
and the underlying circumstances showing that such separation was due to
the legitimate spouses fault.

c) Active member is not yet 65 y.o. (i.e., 64 y.o. and below) at the time of request.
d) No filed application for compulsory retirement.
e) With submitted Designation or Change of Beneficiary/ies Form (signed by the AAO).
