141 One of the most difficult challenges we, as orthodontic practitioners, have to face is
post-treatment retention.
A TORQUING SPRING
We have all seen the type of relapse that happens when we have moved a lingually
placed lateral into proper alignment only to watch the crown begin to move lingually
once again after three to six months of retention. Unfortunately, that is what the
patient sees as well. This type of relapse can happen on any tooth, however, if the
crown was well positioned over the root and the tooth were held in that position for a
time prior to debonding the relapse seen mentioned above may have been minimized
If after the same three to six month time period a bicuspid were to undergo a small
amount of relapse and we offered to retreat that bicuspid, chances are good that
most patients would turn us down. However, in the maxillary or mandibular anterior
segment the patient would be happy to accept re-treatment and would most likely
request it. Typically we then go through either a series of invisalign type appliances,
spring aligners, etc. to correct the problem, and follow this with permanent retention
for as long as the patient will keep the bonded lingual in place.

should talk about how much they love coming to your office.
Sometimes doctors are afraid to appear successful because they fear that the patients will assume they are
making too much money. You have to know your market and understand what is appropriate in your commu-
nity. Thriving doctors, however, know how to appear successful, providing patients with reassurance. Everyone
will assume the practice takes excellent care of their patients.

As a whole, many orthodontists are very sharp dressers. I find it interesting to see orthodontists now wear
very attractive loafers at work. A young orthodontist who just started into practice informed me that a lady
told him he needed to go get a new pair of shoes immediately because all the successful orthodontists she had
ever known had attractive shoes. He took her advice and purchased a pair of Cole Haan loafers. The mature
orthodontist may not have youth on their side; however, they can invest time in a fitness program and their
wardrobe so they continue to portray an image of success.

The mature, successful orthodontist is in an excellent position to create the very best working environment,
excellent salaries, benefits, attractive hours, flexibility, and more vacation time off with pay for staff. When
