Chapter 1
Introduction
Young adulthood, a phase between the late teens and the mid-thirties
(Potter & Perry, 2007), is a time when systems of the body are at their peak and
significant other and commitment to something (Kozier, Erb, Berman, & Snyder,
2004). It is the high time when the individuals dream and plan.

Asia, the majority of individuals affected by chronic kidney disease are young and
in the most productive years of their lives. Progression of chronic kidney disease

Research Methodology

Philippines for his leave, he felt nauseated and had his blood pressure checked
which turned out to be elevated. Later, he was diagnosed with kidney failure
Kate is a 35-year old female, married with one child, who had a kidney
transplant almost two years ago. She busies herself being a wife and a mother
while helping out with her husbands business. She felt ill with fever and
glomerulonephritis at the age of 33. Just a few months on hemodialysis, she was
able to have a kidney transplant. To date, she has good graft functioning and
Rose is a 23-year old female, single, who had a kidney transplant about
six months ago. She is a part-time teacher although she stopped when she was
about to have the transplant. She was diagnosed with systemic lupus
erythematosus (SLE) when she was only twelve years old. From then on, her
kidney function was regularly monitored by her nephrologist. With lupus nephritis,
her kidney function declined and was advised to have a hemodialysis access
placed in her arm when she was twenty-one years old. But she never made use

of this since she had pre-emptive kidney transplantation done. To date, she has

which, data collection was done through semi-structured interviews with the
informants in their preferred venues. Each interview lasted from half an hour to

and note taking was done during the course of the interviews. These interviews

interview. These notes were then coded into key words or phrases that captured

transcript, followed by page numbers for easy retrieval. This process was
repeated as the study moved on to the next cases while remaining open allowing
new themes to emerge with each case. Connections and patterns were searched
subthemes with their corresponding relevant transcript extracts. Taking the final
themes, the narrative account of the study was then written up.

Ethical Consideration
This study adhered to the principle on respect of human dignity and this
The informants had the right to decide voluntarily to take part in the study and
may refuse to give information and may also withdraw his/her participation any
time of the study without penalty. Informants were explained of the purpose and
benefits in joining this study and were given the chance to ask questions and/or
clarifications. Written informed consent was served by the researcher and signed
directly by the informants before the actual study. Informants were informed that
audio recording and note taking are done during the course of the interview.
Specifically in the informed consent, they were informed that they may
stop the interview anytime they felt doing so without any repercussions. A signed
copy of the consent form was given to the informant and a second copy was
filed. The researcher gave the informants the option of reviewing those data
extracts from their own interviews which will be chosen to appear in any public-
domain document. Audio tapes were transcribed and both tapes and interview
notes were stored safely in a locked cabinet which only the researcher had
access. Recordings were discarded three months after the study. The names of
the informants, the people they refer to and location names have been edited to

Thematic Analysis
After analysis, emergent themes were identified which were later clustered
revised or discarded if they did not seem relevant (Smith et al., 2009). After

Likewise, many kidney recipients after having recovered from the operation felt

chronic kidney failure. Almost all had to have dialysis before the transplant and
except for Rose who had a pre-emptive transplantation which means she had the
transplant before initiating dialysis but still did experience symptoms of chronic
kidney failure. They reported felt freedom from these limitations. Andy described

feeling nakadaog ka ug extreme sports, kanang Ironman. Murag ingon ana
imong feeling... (I can now walk without catching my breath. My stomach
does not ache easily. I do not get dizzy easily. I can urinate regularly and
normally. [ ] Having the kidney transplant is really different from undergoing
dialysis. [ ] I do not get easily tired walking. [ ] It is like winning in extreme sports,
like Ironman. It feels just like that)
As for Pam, she felt delighted with the resumption of her menstrual cycle and

Apart from freedom from physical limitations, strict dietary restrictions during
dialysis were also lifted off their shoulders. For example as Bea reported:

a renal failure, I had to stop [schooling] because three times a week I have to
undergo a hemodialysis. So I dont have enough time to study. Nick

freer lifestyle (Orr et al., 2007). Although, not a cure for kidney failure, kidney

Even with a functioning kidney graft, life after transplant is not quite normal as it
was before and this often leads to a life that is very self-regulated to ones own
body (Svenaeus, 2012). Likewise, the informants have placed it upon themselves
to take care especially of their transplanted kidneys. They to do so but there are
times when they fluctuate from readily accepting it to feeling the weight of

Most of these changes were welcome and are greatly appreciated by informants
who compared themselves when they did not have the transplant yet. But drug
...gaanam siya [hair loss]. Pero naa sad nuoy nanubo pero lahi na strand murag
nipis gani siya. (It [hair loss] is gradual. Although there are hair growths but the
strand is different like it is thinner.) Pam
Single and female recipients were troubled by these side effects as this could
mean loss of physical attraction (Kamran, 2014). The female informants were
more vocal about the undesirable side effects and how these negatively affected
their body image. On another note, two of the informants had surgical closure of
their dialysis access after having the kidney transplant. Informants who had it

have it removed]. (It was bulky. The vein engorged. Now its no longer engorged.
[ ] Others who saw it would say, What is with her [arm]? Why is it big?. It was

really big. It bothered me. It is like you cannot believe it is already gone. I am
really happy that it is no longer there. [ ] I really decided [to have it removed].)
Bea
Although this procedure may have its cardiac benefits, it is still debatable (Unger
& Wissing, 2006). For patients, having a fistula to facilitate treatment for end-
stage renal disease undermined their body image and self-esteem (Finnegan-
John & Thomas, 2013). As for the said informants, its removal improved the
appearance of their affected extremities and in turn enhanced their body image.

feeling like it tingles upon touch where it was operated on. It feels like you do not
feel anything, like it is numb, I do not know why. [ ] You could really feel
something is different. Like, when you look at the mirror, its not level [with the
rest of the torso]. The [transplanted] kidney really protrudes.) Rose

buddy. (Something was placed inside here. <points to incision line> [ ] when I

Although most recipients became adjusted already to the presence of the new
organ, they always feel the organs presence even many years after the
transplant (Shimazono, 2008). For the above informants, the presence of the

organ is palpable and reminds them that indeed a new kidney was placed inside
them.
Informants spoke about how they dealt with the things they had to face in
coped well.
Informants had different strategies to cope with the varying stresses they

nimo ana. Murag kanang sa kuan sa psychological [support] gani na nimo na
kanang naay daghan nag-love nimo. Dili ka mawagtang ug hope kay naa pa sila,
ana. (Their prayers alsoYes, it really helped me. And also that moraleven
moral support for me, like that. That You can make it, you can do it, like that. It
is like that psychological [support] of you that a lot of people love you. You do not
lose hope because they are there, like that) Pam
Sense of humor was also used to cope with the struggles of life with transplant.

weeks maam [hair will all be gone] <laughs> [ ] Salbahis lage na kuan [hair
dresser]<laughs> Gi-ingnan kog two weeks na lang. Mao to na-worry ko
kinahanglan nako mupalit ug wig ani. <laughs> The one who cut my hair told me,
Maam your hair fall is really severe, maybe in a matter of two weeks maam [hair
will all be gone] <laughs> [ ] She was a mean [hair dresser]<laughs> I was

told it was only a matter of two weeks. That got me worried if I should buy I wig
already. <laughs>) Rose

Kidney transplant patients, despite the wide range of positive and negative
strategies helps the young adult to effectively cope with the transplant
experience.
Kate and Pam relied on their faith to see them through. As seen in the accounts:
Wala, wala ko naghuna-huna ana. Nobody knows kung kanus-a jud ta. [ ] Naay
mo-ingon na basin 10 years ra tagal but only God knows. Ako lang, hundred
percent I can do it. Silay nakuyawan ako wala. (No, no I am not thinking about
that. Nobody knows when is our time. [ ] Some would say it might just last for 10
years but only God knows. For me, one hundred percent I can do it. They are
anxious but I am not.) Kate
Pray lang gyud ko, ana. Daghan sad ang nag-pray [for her]. Wala lang ni
complicate [transplant outcome] (I just prayed, like that. Many people are
also praying [for her]. It [transplant outcome] did not complicate) Pam
Reliance on belief in God is also evident with heart transplant patients (Peyrovi et
al., 2014) as they face unpredictable outcomes. For the informants, this faith
gives them a stronghold and helps them face uncertainties. On the other hand,
Nick and Bea did express their fear of transplant failure. Nick focused on the
ever that happens while Bea took time letting go of her dialysis access since it
was her safety net in case her transplant failed. The following are their accounts:

months before I had it [the dialysis access] removed. Never mind if that [the

It was Andy who actually dealt with diagnosed rejection a year after the

The young adults endeavor for independence from ones family of origin is
affected by chronic illness since there is time when they do need to rely on their
families (Walter and McCoyd, 2009). Informants were at a time when they were
fledging on their own but had to go back and seek support from their families due
time when interacting with friends were limited because of the reverse isolation
precautions they were under. But later on, interaction resumed when these

...There are friends that I told already since theyre close to my family and Im
close to their family so they know but the college group of friends are not close to
my family and I intend to keep it a secret from them. Nick
as some chose not to divulge while some, after weighing the pros and
embarrassed being different although those who did tell felt relief
(Nicholas et al., 2011). This decision to tell or not to tell friends is vital
issue for the young adults as they managed their transplant condition
status. Four are single while two are in a committed relationship (one is married
and the other one is engaged). Those who are not in relationships shared how

While those in relationships shared how their relationships helped them through

Summary of Findings
The kidney transplant experience of the young adult ultimately altered the
course of their lives albeit mostly for the better as it provided alleviation of chronic
kidney disease. But it is not a quick fix as it also brought new and unique
challenges and problems. Its repercussions are pervasive in every aspect of their
lives. Young adults had to cope with the challenges of post-transplant life on top
of the challenges they faced as young adults. This is a point of struggle as kidney
lives plans and goals. From simply looking at young adulthood as the high time
to dream and plan, they now have to take things into perspective, recast their
course and integrate the kidney transplant experience in their lives. The results
were found to be consistent with existing literature although these studies were
