December Special Events

Resident Holiday/Birthday Party: Wednesday, December 6th, Melissa & Company (bluegrass, country and gospel music) will en-
tertain in the main dining room from 2:00-3:00 p.m. and on Unit 1 at 3:00 p.m. Extra help is needed to set up, assist in transporting to
and from this event and serve refreshments. Hours needed are from 1:00-3:30 p.m.

Santa Gift Delivery: Friday, December 15th, at 9 a.m. until all gifts are delivered. We are seeking
individuals who can deliver and help residents open gifts/label items received, help Santa, sing carols
and visit individually with each resident. Hours needed 9 a.m. until 3 p.m. (you may volunteer for part
or all of the day).

Resident Christmas Store Shopping: Monday, December 18th from 2:00-3:00 p.m. (CHRISTMAS
STORE HOLIDAY GIFTS and GIFT BAG DONATIONS ARE NEEDED by Monday, December 8th).
These items are FREE to Residents so that they may choose presents for themselves, their family,
friends, grandchildren and loved ones. Residents may shop for two to three presents each,
(depending upon how many items we have donated). Volunteers are needed to assist in shop-
ping, wrapping, labeling of these gifts and bringing residents to and from the event. Hours needed are from 1:00-4:00 p.m.
Well also have holiday music and refreshments.

Christmas Eve Candlelight Worship Service: On Sunday, December 24th, Keith Carroll, family and friends will host this special oc-
casion at 6 p.m. in the main dining room. Families are welcome to join loved ones for this event.

New Years Eve Party: Friday, December 29th Chris Foster will entertain us from
2-3 p.m. with Dean Martin music and songs of yesteryear for our New Years party. Extra help is
needed to transport residents to and from the event and pass out party hats and noisemakers. Hours
needed are from 1:30-3:30 p.m.

ADVANCED DIRECTIVES
It is the policy of New Mark Care Center to uphold each individuals right to complete an Advanced Directive, if they do not already have one in
place.
Advance Directives are optional documents that allow you to express your choices about your care in the future. You put your wishes in writing
while you are still able to make the decisions.
When an individual admits to New Mark Care Center, you are asked if you have an Advanced Directive or Living Will in place. If you do not, you
can contact Social Services and they will help you complete one. Social Services will also review with you on an annual basis, your right to make
treatment decisions for yourself. You will also have the opportunity to complete an Advanced Directive.
If you choose to put your wishes in writing and name a Power of Attorney for Healthcare, these records will be kept on file and in your medical chart
for reference.
