Paolozzi and | am a Parole Otticer tor NYSDOCCS. I have been Parole Officer for about 12 years. On 11/10/17 at approximately 1130 AM, I went to the Rome Police Dept, to file 2 criminal Mischief compleint against Sist Respondent Ronald Cook, DOB: 02/09/1951, LKA of 129 Eagle St. room #7 in Utica NY 13501. Ronald Cook is known to me as being under my supervision. Ronald was relecsed from CNYPC on 7/6/17 as an article 10 Sist case. Sist stands for Strict and Intensive supervision and treatment. Immediately upon Ronald Cook's release the GPS monitoring bracelet and the SCRAM alcoao! monitoriag bracelet were installed on Ronald Cook’s legs. On 11/7/17 at ebout 0903 hours, I persozally oliserved both bracelets attached to Ronald Cook's legs and that each were working properly. On Wednesday November 8", 2017, at approximately 1546 hours, NYSDOCCS and I received a notification ‘from Veritracs stating that subject Ronald Cook tampered with his GPS monitoring bracelei. Ronald Cook's GPS monitoring bracelet’s number is 12774322 and is valued at approximately $260.00. After NYSDOCCS and I were notified of the tampered bracelets, # multiple law enforcement agency absconder search was isfinted «C for Ronald Cook. weld ech On November 10", 2017 at about 1015 hours, Senior Parole Officer Fox recovered Roneld Cook's GPS monitoring device from a basement bathroom garbage can at St. Peters church located 200 N. James St. ir Rome N.Y. 13440. Also recovered with the GPS monitoring device wes Ronald Cook's SCRAM aicohol monitoring bracelet # 115256 and an orange handled pumpkin carving knife. The scram monitoring bracelet is valued at approximately $175.00, Each of Ronald Cook's bracelets were cut at the band by what appears to be the pumpkin knife. The total value of the two damaged bracelets is $435.00. I wish to pursue criminal charges on behalf of NYSDOCCS fgp the two damaged ankle bracelets. Ronald Cook was never given permission by myself or damage said ankle bracelets. All is tue and accurate to the best of my knowledge.
