- Student continued to be punctual throughout the
clinical placement during day-to-day fieldwork
as well as with meetings.
- Student continued regular communication with
instructor and preceptors to ensure project
development and course objectives were being
met.
- Student consistently demonstrated a responsible
and respectable presence representing Trent
University Nursing.
- Student completed all assignments on time and
utilized time at the placement and days working
from home well.
- Student continued to ask questions when
necessary to gain more evidence and knowledge
of community nursing, and project planning.
- Student has continued to conduct a community
assessment and organization assessment.
- Student was able to conduct a process
evaluation, but as the database will not be used
until after placement is finished the outcome
evaluation of the database will have to be carried
on by Green Wood.
- Student has gained a comprehensive knowledge
base of grassroots organizations and all of the
community work that goes on.
- Student completed poster on evaluation of the
RedPath program.
- Student completed project note 2 on the
evaluation process of the project plan, and
showed great learning experience.

- Student was very respectful and professional
with the various individuals at the activities,
dinners, and meetings.
- Student has been very respectful towards
preceptors and clinical instructor.
- Student consistently worked within scope of
practice.
- Student has learned about the vulnerable
population Green Wood works with and
understands the importance of respect, knowing
biases, and non-judgmental attitudes.
- Confidentiality was held to high standards by
student throughout placement.
- Student was able to collaborate and respect
group process with Cyndi and Nicole over
creation of the RedPath programs database and
poster.
- Knowledge from previous courses continued to
be deemed useful when trying to demonstrate
relational practice with different individuals in
community.

drug/opioid seminar.

1. I have become more confident in my communication skills and opened up more when engaging with individuals within the community.
2. I have been very productive with my independent work and continuously taken initiative to collaborate with all the parties involved in the
RedPath program, and Green Wood planning.
3. I have become more proficient in my literature review skills and have a better understanding of database creation and the importance of data
collection in community development.

Daijauna presented herself professionally
throughout the placement- she was accountable for
the tasks assigned, followed through in a timely
manner and kept her preceptors in the loop- she
worked well independently and with the
community
Daijauna did literature searches collecting
information needed to inform both the work going
on at Green Wood but also her own learning
-created a data base to support the collection of
data surrounding the Luke4 grant
Daijauna worked from an ethical framework-her
knowledge of social inequity certainly increased
during this placement- better understanding of
how the social determinants of health impact folks
on a daily basis
Daijauna demonstrated respect for all folks
including those who have been marginalized by
the broader community- she has a calm accepting
manner
 meet the needs and build capacity of the population in
the given context.

Daijauna's has experienced point of care social
justice work with GWC- she has developed a
deeper understanding of challenges facing folks
who live in a rural setting with respect to access to
health care services, housing challenges and
addiction challenges- Daijauna has experienced
firsthand the impact of gaps in the health care
system and some of the challenges faced when
trying to meet community health nursing standards

Daijauna has risen to the challenges of working at the point of care in a community drug reduction project. She jumped in to each experience offered
and was professional and engaged in her own learning. Daijauna did well creating a project with minimal guidance that should serve the research
team well in the data collection phase of the Luke4 grant work.
