Dr. Sunitha HOD OBS & Gynae A has already prepared these instructions for Maternity ward (Annexure 1) and need t
printed and given to district hospital. It should be multi-lingual i.e Telugu/ Urdu/Hindi and which shall be filled at all
by doctors / nursing staf/Anesthetist / dietician and kept for future reference, last page of the booklet is a discharge
summary page (perforated) shall be given to the patient.
