JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, JAIME PARAYRAY and OSCAR BRITANICO, both of legal ages

and Filipino Citizens, both with residence and postal address at Catarman,
Northern Samar, after having been sworn in accordance with law, depose and
say:

1. That we personally known the person named JOVENCIO D.

BALLETA, SR. who was born on December 15, 1947 in
Catarman, Norther Samar;

2. That we know that he applied for application for Provident

Benefits in Pag-ibig Fund Office at Catarman, Northern Samar ;

3. That in his application record with the Pag-ibig Office he

mistakenly entered his name as Jovencio D. Balleta instead of
his true and correct name Jovencio D. Balleta, Sr., the former
name being the name of his son;

4. That said mistake in his name was due to inadvertence and honest
mistake and is not intended to defraud his records or third
persons;

5. That we personally known said facts because we are longtime

friends and that we are co-employees of LGU- Catarman, Northern
Samar;

6. That said name Jovencio D. Balleta and Jovencio D. Balleta,

Sr. pertain only to one and the same person;

7. That we are executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have signed this affidavit this 29TH day of

January 2013 in Catarman, Northern Samar.
