a) Hazards can be divided into three main groups, health hazards, safety hazards, and environmental
hazards. Differentiate the hazards in term of definition, example (at least 3) and Malaysia Act and/or
regulations.
b) Figure 5b shows a worker handling the welding proses. To ensure that the welding proses activities
are being carried out in a safe and healthy condition, potential hazards need to be identified,
assessed and controlled. Prepare and generate a HIRARC report by identifying FIVE (5) hazards.
Analyze the risks and suggest the risk controls. (make your own assumption if necessary)

b) Heinrich's Domino Theory states that accidents result from a chain of sequential events,
metaphorically like a line of dominoes falling over. When one of the dominoes falls, it triggers
the next one, and the next as shown in Figure 6b, but removing a key factor (such as an unsafe
condition or an unsafe act) prevents the start of the chain reaction. Generate 5 situations
that lead to start of the chain reaction of domino theory at a chemical factory. For each situation,
propose the prevention action to your management.
