Applicants are required to enclose the following documents with this application:

1. Copy of the Results / Marks Sheets / Transcripts issued either by the Examinations Department or Institute /College.
2. In case of migration/NOC a copy of degree or final transcript/ marks sheet.
3. Copy of Matriculation Certificate.
4. Copy of Enrollment Card.
5. Copy of CNIC Card.
6. Attached a receipt of fee (Rs.500/- as Ordinary & Rs.1,000/- as Urgent) per certificate.

The Certificate for the outstation candidates will be issued in 10 working days @ Rs.500/- per certificate plus Rs.200/-
as courier charges.
For collection of Certificates showing of original Enrollment Card is mandatory.
The Certificate is issued the student concerned, however the same may be collected on authority letter.
