OF JELYNE P. TAGUINOD

I, Jelyne Taguinod y Pagunuran, 34 years old, married, and a resident of #12 Galindon St.,
Bagay, Tuguegarao City witness for the prosecution in this case, states under oath that:

PRELIMINARY STATEMENT

The person examining me is Atty. Maries M. Miguel with office address at Caritan,
Tuguegarao City, Cagayan. The examination is being held at the same address. I am answering
her questions voluntarily, to the best of my knowledge and fully conscious that I do so under
oath and may face criminal liability for false testimony and perjury.

PURPOSE

This affidavit/testimony of witness Jelyne Taguinod y Pagunuran is being offered to prove the
following:
1. The identity of the assailant.
2. The circumstances that transpired during the commission of the offense charged

QUESTION PROPER

3. Q. Please state your name and other personal circumstances for the record.
A. I am Jelyne Taguinod y Pagunuran, 34 years old, married, and a resident of #12
Galindon St., Bagay, Tuguegarao City.
4. Q. where were you on December ____ at around 9:00 in the evening?
A. I was standing at the east west bank waiting for a tricycle to go home.

5. Q. what happened next?


A. There were two (2) men who came along and later were standing beside a motor
vehicle few meters away from me.

6. Q. what happened next?


A. One of the man crossed the street and then there was a shooting incident sir.

7. Q. Do you remember how far the shooting incident from where you were standing?
A. Yes, sir. About 8 meters away.

8. Q. What did you do when you heard the shooting?


A. I ran to the empty police post infront of Eastwest Bank sir.

9. Q. what happened next?


A. People around the place started to run in panic, there was chaos sir

10. Q. what happened next?


A. When I thought that I was safe in my location, I peeked and there I saw a man
wearing bull cap fleeing the scene calmly and riding a motorcycle driven by
Casanova.

11. Q. Was these men the same men you saw standing along East West bank while you were
waiting a tricycle?
A. Yes sir.

12. Q. Did you notice anything peculiar on the person fleeing the scene?
A. The man was about 55, with a mustache, medium built, fair in complexion and
ugly as fuck.

13. Q. Is there anything more?


A. The driver of the motorcycle is about 510, built, fair in complexion and
outstandingly handsome.

14. Q. Is there anything more?


A. Yes sir, the motorcycle, as far as I remember, is a Suzuki Raider with plate number

15. Q. What happened next?


A. People started gathering around a wounded person purposely to help him because it
seems that he sustained injuries.

16. Q. What happened next?


A. The police officers from Tuguegarao City, Police Station came and cordoned the
crime scene.

17. Q. What did you do after?


A. I came near the police officers and I told them that I saw the events that transpired
therein

18. Q. What happened after?


A. The officers took my statements as part of their initial investigation and told me that
they may call me anytime if they may need my assistance.

19. Q. Is there anything more you would want to add?


A. No more sir.
