The company Desarrollo Salaverry 475 SAC, has established as objectives of the
organization to offer its services with adequate safety and efficiency standards, in order
to be competitive and profitable. It considers that its most important capital is the
workers, for which it is the company's priority to maintain good safety and health
conditions at work, as well as to keep the staff motivated and committed to the
prevention of work risks, for which it has established the following policy:
Protect the health and safety of workers, as well as users, visitors and contractors
(natural or legal persons).
Comply with safety and health regulations at work applicable to our activities.
Provide continuous improvement of our performance in risk prevention,
implementing a health and safety management system at work, through which,
all company workers are involved in the continuous identification of hazards and
evaluation of their risks in order to take timely and effective measures to control
them.
Promote and motivate in our personnel the prevention of work risks in all
activities, through communication and participation and measures to control
them.
Promote and guarantee the conditions of safety, health and physical, mental and
social integrity of workers during the development of work in the workplace and
in all those places that are commissioned for the need of the service, being one
of its objectives principal to avoid risks and accidents at work, as well as
occupational diseases.
Ensure that workers and their representatives are consulted and actively
participate in all elements of the Occupational Health and Safety Management
System.

7. Responsabilities

Project manager:
Provide leadership in the planning, organization and control of the work effort to
achieve the established objective.
Coordinate the activities of the different team members to ensure that they
perform the right tasks at the right time and work as a group.
Involves the project team in these functions to make them commit to the
successful completion of the project.

Resident Engineer
Know in full the scope of the work contract.
Maintain and safeguard in the work the documents that are required during the
execution (Plans, Records, Descriptive Reports, Specifications,
Communications, etc).
Ensure that the dates stipulated in the Planning of the work are met as planned.
Comply with the process, control, billing and monitoring of the valuations of the
play.
Ensure strict compliance with the rules of hygiene and occupational safety of the
work.

Supervise the Quality of the Materials and Equipment to be used on the
construction site.

Boss of technical office


Manage the availability of information (Technical File) for the person in charge
of the field engineering process.
Review and validate the consistency of the information of the plans generated by
the subalterns
Manage the availability of information (Technical File, field information,
executed, additional budgets, among others) for the person in charge of metrics
Ensure that the measurements are made in accordance with each objective
(determine the actual initial amounts, additional budgets, determine the amount
of work performed, settlement and valuations)
Ensure that the amounts actually executed in the recovery period are quantified
Corroborate that the quantification calculations are consistent, reconcile the
amounts with the Supervision and present the valuation.
Verify that the initial terrain conditions are consistent with the Technical File
Ensure that the survey, trace, staking and control are executed correctly
Ensure that the survey of the constructed works is in accordance with the
technical file
Deliver the plans for construction to the production staff
Manage technical queries with Supervision and / or Customer
Make a list of permanent materials and equipment and their corresponding

Quality engineer
Identify and analyze the problems related to Quality Systems.
Implement, design and develop Quality Control systems.
Perform audits of the Quality Management Systems.
Implement the different Quality tools.
Prepare certifications and accreditation of Quality Systems.

Supervises the processes of corrective actions of each one of the suppliers.
Provides knowledge to control and / or analyze quality problems and solve them
by implementing solutions.
Is responsible for leading the teams related to the management of the measures
corrective measures necessary to ensure that non-conforming conditions of
quality do not repeat themselves.
Defines, approves and allows the application of the Quality requirements, plans
of quality and quality control points.
Develops and implements the Quality Management System.

Safety Engineer
Preparation and execution of occupational health and safety programs.
Implementation of occupational health and safety system.
Preparation of the annual Occupational Health and Safety plan.
Management of management indicators.
Responsible for training on occupational health and safety issues for company
personnel.
Identify, evaluate and analyze the hazards and risks that have an impact on job
security.
Recommend preventive and corrective control measures for safety risks and
Occupational Hygiene.
Define and establish security standards in critical tasks.
Conduct investigations of incidents (accidents and near misses) of work that
occur during the execution of the service, also proposing corrective measures.
One of its main functions is to evaluate the equipment and processes for find potentially
risks areas Through periodic inspections and observations to the work processes, they
identify defects that have potential to cause damage or injury.

Warehouse Manager
Address of merchandise entry and exit operations.
Verification of fulfillment of order forms, making sure that the planned
procedures are met over time, with quality and security planned.

Decides on inventory control procedures and supervises its fulfillment. In

They will be done on a weekly basis depending on the type of indicator, comparing
scheduled inspections with those that have been carried out effectively in the month. To
be able to evaluate the effectiveness of the inspection program compared to the other
months.
In addition, it will allow observing the actions and work areas that require greater
attention and above all those that present the greatest risk or security.

AFILT: Accident Frequency Index with Lost Time


With this indicator it will be possible to measure the number of work accidents
performed for a month. The person responsible for measuring accidents will be the risk
prevention

Where the value of the indicator must be greater than 5 H. cap. x month x worker

FIEI: Frequency Index of Environmental Incidents:
With this indicator it will be possible to measure the frequency of environmental
incidents
performed for a month. The person responsible for measuring accidents will be the
risk prevention

AFILT: Accident Frequency Index with Lost Time

This indicator will be able to measure the number of accidents with Time that
performed for a month. The person responsible for measuring accidents will be the

SII: Security Implement use indicator


With this indicator you can measure the use of personal protective equipment
that are given to the workers: leather gloves, plugs and lenses in
work, through an inspection of EPP which must be accredited in a register.

The person in charge of keeping the register to find the IIS will be the warehouse
manager or
your assistant
Likewise, this indicator will allow to evaluate the communication between the members
of the organizational structure of the company and the fulfillment of their established
responsibilities.

THI: Training Hours Indicator

It allows to compare the hours used in training during the execution of


the work regarding hours of work in which the training is not carried out. It is a measure
of compliance with the Training Program indicated in the safety plan, as well as having
control over the progress made in the implementation of the plan, given that it is one of
the elements of the Safety, Health and Environment Plan.

The IHC is obtained as follows:

10. Security standars


The following chapter consists of the description and elaboration of standards and work
procedures established by the G.050 standard: Safety during construction. Before the
elaboration about the security standards, it will be necessary to identify all the activities
to be carried out on the construction and the hazards linked to each one. This in order to
be able to propose preventive measures and reduce or even eliminate their probability of
occurrence on site.
Once all the possible risks in the work of the activities have been identified, they will be
classified according to their magnitude, in other words, greater importance will be given
to those that are critical, prioritizing them and looking a solution as soon as possible.

Some of the safety standards for the Salaverry Residential and Commercial Joint
Project are the following:

With the identification of the activities and risks that were identified and classified in
the IPER matrix, the operational control matrix (IPERDC) is drawn up in which the
critical activities will be identified in order to apply prevention or control measures.
The considerations to consider are the following:
Documentation of the procedures to be carried out in order to maintain politics
and objectives of the security plan.
The procedures will be known to all the influential participants in the project.
The realization of the mentioned matrix will control and monitor the risks and probably
minimize or eliminate them. It will also consider the training of staff regarding the
existence of risks in the project and how to develop their work avoiding the occurrence
of these.

12. Construction procedures

In this chapter we considered the procedures or activities which have the greatest
risks over the others. Among these are mainly those during the construction of the
hull of the building that would be the concrete pouring, placement of steel,
formwork and stripping, work at height, earthworks and welding work.

a) Concrete pouring
Objective: Establish safe procedures during the emptying of concrete in order to avoid
accidents during the execution of the activity.

Scope: Directed to people that are in the activity like workers and subcontractors.
Responsibilities: The person in charge of the inspection in the area where the emptying
is taking place will be the foreman who will verify that the established standards are met
such as safety distances, signs, etc. Operators must perform their duties with the
minimum clothing required, in this case, complete personal protection equipment.
Procedures:
Join the mixer to a specific area.
Ensuring the immobility of the mixer with wooden blocks placed on the wheels.
The emptying is carried out when everything is satisfied according to a
supervisor who must be present.
Addressing the mixer hose by an experienced and qualified operator and
reception with the buggies to be transported to the emptying area.
Testing as settlement, density and others and extraction of material for
compression tests.
Procedures to consider:
The foreman or the person in charge should check the emptying areas and verify
the correct condition so that he can receive the mixture.
The minimum safety distance of 2m with respect to the edge of the emptying
shall be indicated by a line.

b) Steel placement
Objective: Establish safe procedures during the placement of the steel in order to avoid
accidents during the execution of the activity.
Scope: Directed to people that are in the activity as workers and subcontractors.
Responsibilities: The person in charge of the inspection in the area where the steel
placement is being carried out will be an assigned foreman who will verify that the
established standards are met such as safety distances, signs, state of equipment and
materials, etc. Operators must perform their duties with the minimum clothing required,
in this case, complete personal protection equipment.

Procedures to consider:
The stacking height of steel rods should not exceed 50 cm and walls or
protective barriers should be placed to prevent them from falling.
The tools to be used, such as the shear, hammers, griffin for bending must be in
optimal conditions for its use, which will be approved by an assigned supervisor.
The workers to manipulate the tools and to perform the steel placement activity
must have experience and be qualified.
The shear should be blocked when it is not in use.
The traps for bending must be resistant and stored at the end of the working day.

c) Formwork and stripping


Objective: Establish safe procedures during formwork and demoulding in order to avoid
accidents during the execution of the activity.
Scope: Directed to people that work in the activity as workers and subcontractors.
Responsibilities: The person in charge of the inspection in the area where the formwork
and demoulding is being carried out will be the foreman who will verify that the
established standards are met such as safety distances, signs, quality, etc. Operators
must perform their duties with the minimum clothing required, in this case, complete
personal protection equipment.
Procedures to consider:
Daily training of five minutes before starting activities.
The material of the formwork either wood or metal should be in optimal
conditions such as clean, protected from moisture, dry, etc.
For the wooden formwork, the workers in charge of cutting and preparing the
pieces must be qualified to perform these tasks.
For work with electrical equipment such as electric saws, it will only be handled
by trained and authorized workers.
Big panels should always be transported by at least two people.
For the transportation of wooden or metal formwork, the staff must provide
leather gloves.
Any nail protruding from the formwork must be removed or riveted.

d) Work at height
Objective: Establish safe procedures that can be fulfilled for work at height in order to
avoid accidents during the execution of the activity.
Scope: Directed to the workers that are in the activity as workers and subcontractors.
Responsibilities: The head of the project will be responsible for delivering the necessary
resources to perform safe work at height. The foreman or supervisor will be the one in

charge of inspection in the work area and recommendations for the execution of
activities in height.
Procedures to consider:
Before for the execution of the activity, it is necessary to establish procedures to
be followed and measures of security protections such as epps or epcs.
To prevent falls, the area where there may be a probability of falling, such as
building edges, elevator boxes, etc., must be defined.
The passage of workers other than the activity to be carried out at height must be
prohibited.
The use of harness will be mandatory for cases in which a possible fall is bigger
than 1.8 m on the floor and 1.5 m away on the edges of the building.
Harnesses and hitch lines will be inspected before being used.
For scaffolding work, a construction made of it and in optimum condition to be
used will be taken into account. Its structure should have cross braces or full
lateral braces and well fixed.
The scaffolds must be properly secured at the base and according to the lateral
elements when the height of these structures reaches three times the dimension
of the shortest base.
For mobile scaffolds, their movement must be blocked during use and must not
exceed three height bodies or be used on inclined surfaces.

e) Earthworks
Objective: Establish safe procedures during the execution of activities related to the
movement of lands in order to avoid accidents during the execution of the activity.
Scope: Directed to people that work in the activity as workers and subcontractors.
Responsibilities: The person in charge of the inspection in the area where works such as
excavations or fillings are being carried out will verify that the established standards
such as safety distances, signs, etc. are met. Operators must perform their duties with
the minimum clothing required, in this case, complete personal protection equipment.
Procedures to consider:
The field engineer will verify the bracing of the surrounding structures since the
excavation of the land could cause damage.
It will be necessary to verify the electrical and sanitary installations through the
plans of the neighboring homes in order to avoid consequences that may cause
material damage and accidents to people.
The owners of the nearby structures will be notified about the excavation to be
carried out soon as well as the preventive measures to be taken.
"Danger deep excavation" signs will be placed in points of the perimeter fence.
Workers who are working on excavated land must be a minimum distance of 1.8
m from the walls of land.

f) Welding works
Objective: Establish safe procedures during welding work in order to avoid accidents
during the execution of the activity.
Scope: Directed to people that work in the activity as workers and subcontractors.
Responsibilities: The person in charge of the inspection in the area where the welding is
being carried out will be the foreman who will verify that the established standards are
met such as safety distances, signs, protective equipment, etc. Operators must perform
their duties with the minimum clothing required, in this case, complete personal
protection equipment.
Procedures to consider:
The welding operator must use at all time his personal protective equipment,
such as gloves, helmets and special lenses for the work to be done.
The operator will also use respiratory protection due to the metallic fumes that
may be generated.
It will be necessary to enable extinguishers located near the welding works.
It will be of total prohibition smoking while welding or when they are
manipulating the lighters.

13. Definitions

a) Personal protection equipment (PPE)

Leaded to the equipment that will be used in a personal way by all the workers on site
depending on the activity to be carried out and will serve as protection against the
existing dangers in the execution of activities.

Some of the equipments are:

Safety helmets
Steel toe boots
Protection glasses
Protection gloves
Reflective vests
Earmuffs
Harness

b) Colective protection equipment (CPE)

Those equips assigned for the collective protection of workers with the aim that both
staff and public are protected against existing dangers.

Some of them are:

Rigid railings
Alarms
Horizontal and vertical lifelines
Fixed systems
Switches for electrical boards
Strobe lights on heavy machines

c) Non conformities
It is the breach of the standards or specifications given at the beginning for the project
activities and working conditions.
d) Corrective measures
Actions taken after finding a non-conformity which should correct the current non-
conforming situation.
e) Preventive measures
Action taken to prevent the occurrence of non-conformities.
f) Report of accidents and incidents
Report that aims to record the accidents and incidents that occurred as well as the
identification of the causes that generated them and their corrective measures.

14. Trainings
The objectives of the workers training for the present project are the following:
Make to know the responsibilities of the staff in relation to compliance with the
security plan.
Train the line of command (managers, supervisors, foremen, etc.) in the use and
application of the security plan so that they can be an example to others for
compliance.
Create awareness among staff about the importance of compliance with the
procedures and standards of the safety, health and environmental plan.

Training elements:

Monthly meeting:

The progress of the program will be analyzed in order to correct and control compliance
with preventive and corrective measures. The duration is approximately two hours and
the person in charge is the general manager.
Daily training:
Before starting the work the master builder or foreman will gather the workers to
quickly analyze the tasks of the day, possible risks and determine preventive measures
and safety implements that will be used. The duration of these talks will be five
minutes.
Weekly training:
The resident engineer or construction teacher will give a training once a week to the
workers, in which topics such as the prevention of labor risks, norms and established
laws as well as the analysis of work procedures will be discussed.
Inductions to new workers:
Directed to new workers who will be informed about the importance of safety for the
company. Likewise, a document titled "Compliance Commitment" is released, which
commits the worker to carry out their work safely. The duration of this activity is
approximately one hour and is carried out by the field engineer.
Specific training:
Directed to workers who perform high risk jobs or in special cases. The person in
charge of the training will be the specialized person hired. Also, the duration of this
activity will be two to three hours depending on the complexity of the operation
