ACCOMPLISHMENT REPORT

FOR THE MONTH OF OCTOBER 2011

October 3-7, 2011

Collected and compiled our accomplishment reports and DTR ready for submission.
Attended the Generics Expo 2011 at People Center, Tacloban City (October 4, 2011). I was
tasked to give an intermission number.
Attended the 22nd National Statistics Month Celebration at Robinsons Place Tacloban City
(October 5, 2011) and I was also tasked to give an intermission number.
Received letter from LEFADO c/o Mrs. Betty Garrido (Executive Officer) regarding her
proposal- Training of Guidance Councilors of 20 institutions in the region regarding
HIV/AIDS and the different Sexually Transmitted Infections.
Analyzed and interpreted datas of the Monthly consolidated Sexually Transmitted Infection
reports.

October 10-14, 2011

Entered on the computer the collected Clinic Monthly Consolidated STI Report of each
reporting units in the Region for the month September 2011.
Called Maam Grace Campo regarding the Monthly Consolidated Report of Calbayog Social
Hygiene Clinic and was able to talk with Dra. Romano regarding the concern.
Analyzed and interpreted datas of the Monthly consolidated Sexually Transmitted Infection
reports.
Made my Sexually Transmitted Infection Report for the month of September 2011.

September 17-21, 2011

Performed active case finding among the hospitals in Tacloban City.


Called Ormoc Social Hygiene Clinic regarding their monthly consolidated reports for the
months of January-September 2011.
Called Kananga Social Hygiene Clinic regarding their monthly consolidated reports for the
months of January-September 2011.

September 24-28, 2011

Made graphs and tables regarding the STI cases in the region for the months of January-
September 2011 for the RESU Bulletin Board.
Went to LEFADO to coordinate with Maam Betty Garrido regarding her proposal. We
discussed as to what universities or institutions will be included as participants for the
training and the board and lodging.
Went to DILG to see Maam Juliet Mutya, RAAT Member and gave her feedback regarding
the proposal of Maam Betty Garrido.
Went to LNU to meet with Maam Lilibeth Fallorina, RAAT Member the upcoming forum oh
HIV/AIDS in their instituition.
Received a text from my Co-surveillance Nurse regarding a case of HIV at EVRMC.
Conducted hospital rounds at Eastern Visayas Regional Medical Center (EVRMC) to monitor,
collect, and validate data.
