Career and College Readiness

Student:

Visual Learner, Interests in sports, religion, helping peers. Strengths; motivated, strives for goals,
I always give 150% to everything I do. I never give up, throw me anything, and I'll take it on.
Student has been described as a very charismatic, friendly, hardworking peers. He is very
independent and respectful so he is able to build strong relationships with teachers and peers. At
times he has a tendency to become overwhelmed by the amount of work that he has for his
classes and benefits from de-escalation routines like taking deep breaths and taking a small walk
to clear his mind. Student is not afraid to advocate for himself and will use the accommodations
that are on his IEP. At school he is involved in Track and Field and Link Crew where he is a
positive influence in both.

Areas of Strength:

Student wants to perform well and would like to attend a 4 year institution after graduating from
high school. He struggled his first couple of years in high school but academically Student is
now able to comprehend grade level text and write essays that meet grade level requirements. In
math he is able to solve complex math problems but will get overwhelmed

Areas of Weakness:

Student is easily distracted and needs to feel that he is begin held accountable to a strict timeline.
However he struggles to create and follow through with the time table that he sets for himself

Personal Goals:
Student would like to maintain a 3.5 grade point average or better

Student wants to create a list of colleges that he is interested in and complete college applications
on time.

1. By the end of September Student was able to take the Second meeting
Student will have signed up SATs on October 8th and has
for the SATs and ACT to be received the fee waiver so that
taken by the end of October he can sign up for the SATs
and ACTs in November.
