Kiki (nurse 1) : My name is Kiki, Im from emergency room. Im going to hand-over. She is
Mrs. Pitri.

Sulaimah (nurse 2) : Hai Mom. Im Sulaimah. I will take care you in this afternoon shift. What do
you feel, Mom?

Pitri (patient) : I feel a little pain in this area

Sulaimah (nurse 2) : Ohh, you can take a deep breath to decrease your pain. Ok?

Kiki (nurse 1) : Mrs. Sulaimah, we will start hand-over this afternoon. I need about 5 until 7
minutes to talk to you about this patient conditions. To know about changes to a
patient status, treatment plan, prosedure and enviromental issues related to
patient care

Sulaimah (nurse 2) : Ok. Im ready to hear. Who is she and what her diagnosed?

Kiki (nurse 1) : The patients name is Mrs. Pitri, 55th years old, RM 0909-1005-001. The
patient diagnosed hypertention. Her doctor is Dr. Henry. She has admitted on
today, 03th Nop 2017 at 10.00 a.m. IV line Ringer Laktat 20 bpm already
installed, and the drugs is captopril 12,5 mg at 10.00 a.m according from Dr.
Henrys advice. She doesnt have allergies to drugs or certain foods.

Sulaimah (nurse 2) : Ok. She has been taking captopril at 10.00 a.m? Is it true?

Kiki (nurse 1) : Yes, its true.

Sulaimah (nurse 2) : How about Mrs. Pitris vital sign?

Kiki (nurse 1) : Her blood pleasure is 140/100 mmHg, her pulse is 76 bpm, her temperature is
37,30C and her respiratory rate is 24 bpm. Her pain score is 7, GCS: 15 E: 4 V: 5
M: 6. She havent problem in defecate and urinate. Her diet is regular diet

Sulaimah (nurse 2) : Ok.
