WAIVER, RELEASE AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

hereby absolutely and forever waive, release, discharge and quitclaim any and all rights, claims,
interest, causes of actions of whatever nature which I now have or may have by virtue of the
termination of my Service Agreement with [EMPLOYER], be they civil and/or administrative,
and by virtue of the payment and conditions herein made, I hereby bind myself, my heirs,
administrators, successors and assigns to this quitclaim and release which may be pleaded in
bar to any action of whatever nature which I may institute or might in the future institute against
[EMPLOYER], its principals, its subsidiaries or affiliated companies, members, officers, trustees
or employees, arising from my above-mentioned claims.

I hereby state further that by virtue of the payment and conditions herein made, I have
no claim, right or action of whatsoever nature, whether past, present or contingent against
[EMPLOYER], or its principals, its subsidiaries or affiliated companies or its successors,
assigns or any of its members, officers, trustees or employees.

I furthermore state that I will not testify in any proceeding that may be filed for and in my
behalf by anyone against [EMPLOYER], its principals, its subsidiaries or affiliated companies or
its successors, assigns or any of its member, officers, or employees by virtue of my consultancy,
separation or termination therefrom.

I hereby state that I have read and understood the foregoing Waiver Release and
Quitclaim.
