Plan provides basic life, long term disability, critical illness, extended health care and dental insurance coverages which include features
not typically available to small firms, such as:
major restorative dental vision care orthodontia (some limitations apply) health care spending account
The Firms plan is available to you, even if you have only one insurable life in your Firm.

60% of your monthly earnings to a maximum of $5,000
You can choose to add optional life coverage to your Firms
Choice of taxable or non-taxable

insurance. 50% of your monthly earnings to a maximum of $5,000
