Pt is 78 y.o male who presents to the ED for evaluation of high blood pressure.

Pt states he has
associated HA and dizziness. His sx is aggravated upon standing up or ambulating. Pt denies vertigo,
nausea, or vomiting. Pt reports having intermittent CP, but only when sitting down. Pt states he has not
taken his Lisinopril medication today or the past week.

Pt is a 22 yo with hx of gonorrhea who presets to the ED for evaluation 3 day history of dysuria. Pt states
her sx began after having unprotected intercourse with a different partner. Since having intercourse
with her boyfriend, he has the same burning sensation when urinating. Pt rates her intermittent pain as
a 6/10. She states she has associated nausea, abdominal pain, and cloudy, smelly***vaginal discharge.
Pt denies any vaginal bleeding. Pt had contracted STD in the past and was prescribed doxycycline

Pt is a 99 yo male with hx of dementia, CAD, and HTN, who presents to the ED for 3 day history of
increased confusion. His nursing home states pt has been unresponsive with decreased concentration.
They deny the pt has any fevers, chills, N/V/D, or urinary symptoms. Per nursing home, pt does not have
an appetite and has not had a full meal in 2 days. They notes pt had similar sx the last time when he was
diagnosed with hyponatremia.

***poor oral intake x2 days

Pt is a 55 y.o male with hx of CAD with last ECHO on 11/12/2106 (EF15%) and is currently on a
defibrillator who presents to the ED for 1 day hx of constant CP. Pt states his progressively worsening sx
started when he was mowing the lawn. He denies any nausea, vomiting, or diaphoresis.

Pt is a 42 yo female with a hx of diverticulitis who presents to the ED for 1 week history of LLQ
abdominal pain. She reports of having associated n/v/d and hematochezia. Pt notes she her sx feels
similar to her last diverticulitis flare. Three months ago, pt had seen Dr. Strain (GI) for a colonoscopy that
showed erosions in the sigmoid colon and diverticulosis. She also notes she ate cashews two days ago.
