Sir,

1, Shri Tushar Sashikant Chawate am the member of Voltas Employees Co-operative Housing Society
Ltd, Housing flat C-12 and measuring Sqf on the third floor of the building number/ named
as C-12 of the society.

I propose to sub-let/ give on leave and license /caretaker basis my flat/ part of the flat to the person
named herein afer under an agreement ( copy enclosed) under the following circumstance.

I am required to shif my residence to Thane as part of my official engagements with my employer for
approximately two years. During this period of 2 years, it will not be possible for me to occupy the flat/
continue inoccupation of the flat.

The name of the person to whom the flat/ part of the flat is proposed to be sublet/ proposed to be given
on leave & license / caretaker basis is Sri Rajesh Kongassery & Sumitha Notthankandath residing at
Gokul, Thottekkattuthara, Koduvayur, Palakkad, Kerala- 678501, having office address at IBM India Pvt
Ltd, 4th Floor, Bandra Kurla complex, Mumbai-51. Their application for nominal membership of the
society in the prescribed form is enclosed here with.
I undertake to initiate proceedings against the sub lettee/ licensee/ caretaker on my failure to secure
vacant & peaceful possession of the flat from the sub lettee/ licensee/ caretaker on expiry of the original
or extended period of subletting/ license/ caretaking and to make the society the part to the
proceedings & to reimburse the expenditure which the society may be required to incur on legal
proceedings to get back the possession of the flat/ part of the flat.

I also undertake to pay all the charges of the society every month with in the time stipulated without any
excuse from the date I pay with the possession of the flat/ part of the flat till I get the possession there
off.

I also under take to pay non occupancy charges at the rates fixed under the bye law number 43(C) during
the r mentioned above.

I request you to please grant the necessary permission to sublet / give on leave and license / care taker
basis, the flat/part of the flat to the person herein above mention for the period of two years. In case it
becomes necessary for me to extend the period of subletting/ giving on leave & liscese/ care taker basis,
the flat/ part of the flat, I will submit an application for extending the period well in advance of expiry of
the original period with due justification for extending the period.
