Dear students,

Please read the scenario below carefully.

Scenario

You are conducting a field service of HIV prevention program and management in
Bandung Municipality which is a city with the highest burden of HIV in West Java.

Since 2010, Indonesia has expanded the HIV testing by performing voluntary
counseling and testing (VCT) and provider-initiated counseling and testing (PITC) at
many sites. However, the uptake of HIV testing remains low particularly among key
populations such as female sex workers, injecting drug users, male who have sex with
male and high-risk men. There are several reasons why the uptake of HIV testing is still
low among key populations namely inconvenience of testing location, testing hours, and
stigma.

According to national HIV guideline, ELISA/ Western blot algorithm was used to confirm
HIV infection diagnosis. Despite the benefit of using whole blood samples, oral fluid-
based HIV tests also offer several advantages because it is less invasive, less
hazardous and can be used for self-testing.

As HIV program manager, testing the performance of oral fluid-based HIV test in
comparison with blood-based rapid test is crucial to the success of your program.

Recently, OraQuick Advance Rapid HIV-1/2 test has been offered by a sponsor who
can provide the test in the field, but you are not sure if this test suits your need.

To answer your doubt, a research colleague has searched the internet and found the
journal (see attachment) which title is “Field Evaluation of Diagnostic Accuracy of an
Oral Fluid Rapid Test for HIV, Tested at Point-of-Service Sites in Rural Zimbabwe”
(AIDS, Patient Care and STDs, 2009)

Here are things that you need to do:

1. Formulate your answerable question based on the scenario above according to


PICO scheme.
2. Read the article and carefully appraise it according to appraisal worksheet for
diagnostic study (see attachment). Decide if the OraQuick Advance is as good as
ELISA as screening tool for HIV in the field.

3. As a group, you need to make a summary about your critical appraisal results in
power point presentation in English. One of the group member as a representative
will present the results in 15 minutes. Please bring your own laptop (one laptop for
one group). We will grade your presentation and the score will contribute to your
final CRP score. The file of your group presentation must be copied in CD and
collected to your group tutor before the discussion begin. Should any groups fail to
prepare their presentation on time then that group is not eligible to attend the class.
