AFFIDAVIT

I, ANTHONY BAJO NAVAJA, JR., Filipino, of legal age,

married, and a resident of Barangay, Eastern, Wao Lanao del Sur,
after having been duly sworn to in accordance with law, do hereby
depose and say:

1. That I am married to Josephine B. Bajo on March 28, 2011;

2. That we were blessed with three (3) children, aged 12, 10 and 8;

3. That my wife Josephine left our conjugal dwelling sometimes in

2012 and never come back;

4. That I have never heard of her again neither have I receive any
communication from her;

5. That until today, my wife’s whereabouts remains unknown even to

her parents.

6. That I execute this affidavit to attest to the truthfulness of the

foregoing statement and for whatever worth this may serve best.
