1.0 INTRODUCTION

SPM Melaka is located about 7 km from shore and it has 2 strings of floating hoses.
Floating Hose onshore inspection was scheduled for every two years for each string as
per Specific Equipment Reliability Program (SERP). SPM Outer string has 29 numbers of
hose and Inner string has 28 numbers of hose.

2.0 SCOPE OF WORK

2.1 General

The Work shall be completed within available window (normally 12 days window available
between 2 tanker operation) provided by SUPSB and shall be carried out 24 hours around
the clock.

2.2 Scope by Contractor’s

Contractor shall provide all resources (manpower, tools, equipment, consumable, etc) to
ensure successful completion of work which include, but not limited to the following:

6 Disconnect Floating Hose String - SUPSB OSRE will be deployed and
equipped as precaution
7 Tow floating hose string from SPM to TPJ - SUPSB will provide a mooring boat
- Max. towing speed = 3 knots
- The hose will be tow from F1
8 Dismantle floating hose string at TPJ - Work to be conducted continuously 24
hrs
9 Perform hose cleaning at washing bay
10 Perform MBC cleaning
11 Perform hose patching
12 Perform hose testing at TPJ - Hydrotest
- Vacuum test
- Continuity test
As per PPMSB/ OCIMF guideline
13 Assemble the new string of floating hose at TPJ
14 Tow the new floating hose string from TPJ to SPM - Max. towing speed = 3 knots
- The hose will be tow from F1
15 Reconnect floating hose string
16 Full system hydrotest - To be conducted as per approved

3.0 GENERAL STATEMENT


Serious accidents have occurred and continue to occur to personnel entering or working in
confined spaces. It is SUBSURF SDN BHD requirement that before any clearance for work to be
carried out involving entry into confined spaces, a Hazard Analysis should have been undertaken
to manage the nature of hazards and risk, the necessary precautions for safe entry into confined
spaces whether for inspection, hot work, cold work, painting, cleaning etc. by personnel who are
directly involved in the operations.

This procedure applies to any confined space entry within MRCSB refinery premises. A person
whose body or partly body enter within a confined space is considered to have entered the
confined space.

The following confine space is located at Melaka SPM and are identified as follows.
1. All 6 numbers of CALM Buoy body compartment.
- Where HPU and PLEM valves control unit is located.
2. CALM Buoy moonpool/centre well.
- Where water draining, close/open butterfly valve & rigging works will be performed.

4.0 PURPOSE
The purpose of this procedure is to ensure that:
• Entry into confined spaces is avoided where possible.
• When entry into confined spaces is unavoidable, all hazards have been considered and there
are sufficient safe systems of work and emergency arrangements in place that will reduce
the risk of injury to the persons involved.
• Ensure compliance with relevant legislation.

5.0 REFERENCES

6.0 DEFINITIONS
A Confined Space can be any space of an enclosed nature where there is a risk of death or serious
injury from hazardous substances or dangerous conditions (e.g. lack of oxygen, toxic or gas vapor).

7.0 HAZARDS IN CALM BUOY CONFINED SPACE


Since the line pipe has been flushed with sea water for flanges breaking, there is almost certain
that there will be no oil inside the line.

9.0 PROCEDURE

9.1 Pre-Entry Preparation


Prior to enter into confine space, the following Pre entry preparations has to be done.

• Declaration of fitness by personnel who assign for entering confine space.


• Job Hazard Analysis covering confine space entry.
• Fill up confine space checklist.
• Implementation of ventilation plan.
• Rescue plan has been made aware by the team.

9.2 Equipment Preparation


Any potential danger identify in confine must be isolated by means of Positive Isolation, activities
such as blinding, disconnecting, spading or blanking of all lines connected to the space, or by other
approved means.

9.3 Open Manhole / Compartments

2. CALM Buoy There will be one access for personnel entry with monkey ladder
compartment (0.6 meter diameter) and one enlarge manhole (1.2m x 0.69m)
(HPU compartment) for equipment entry.

9.4 Ventilation
It is the responsibility of the Entry Supervisor to ensure that air-testing requirements to be carried
out by a competent person (AGT), who is trained in the use of the equipment and can interpret
results.

One unit of electrical drive blower (82m³/min air flow rate) for ventilation will be used.

CALM Buoy Compartment - Both access for manway and equipment manhole will be open prior
entry. Once personnel enter into the compartment, continues ventilation using blower at the
manway is to be done.

CALM Buoy moopool/ center well - Both access for manway will be opened and one way will be
having continues ventilation using blower.

9.5 Gas Test


The atmosphere both in and around the confined space must be tested before entry to determine
if entry is permissible. Monitoring should be repeated at regular intervals (as specify in PTW) while
work is in progress and always after a work break.

Gas tests must be carried out in such a way that the result obtained is representative of the
condition of the entire space paying particular attention to locations where toxic or flammable
gases may accumulate; e.g. sumps. Testing should be completed without entering the confined
space, using extended probes. If entry is required for testing, AGT should wear breathing
apparatus.

SSSB will deploy one qualified AGT as standby for confine space entry work.

9.6 Permit-To-Work (PTW)


Entry into confined spaces must be subject to a written permit to work to authorize entry should;
• Describe the work to be performed.
• Job Hazard Analysis to be attached.
• Specify positive isolation and testing requirements and other conditions needed to
perform the job safely.
• Obtain the appropriate signatures as required in the MRCSB procedures.

The duration of the permit to work shall be stated and shall not exceed the time required to
complete the assigned task or job identified in the permit to work in accordance with the purpose
of entry.

A copy of the permit should be available at the work place while the job is in progress. The
Approving Authority must accept responsibility for ensuring that the conditions of the permit are
maintained.

9.7 Entry and Perform Work


Entry under gas free/respirable Condition – Without Breathing Apparatus. A confined space must
be made SAFE (i.e. non-toxic, non-flammable, nonasphyxiate) for persons to enter and work in.

Entry without using appropriate breathing apparatus should only be permitted subject to the
following conditions:
• Oxygen content: minimum 20% vol, maximum 20.9% vol
• LEL reading: < 1% of LEL
• Toxic vapours: not greater than 50% of the OEL

9.8 Close Out


A hand-back signature must also be completed on the permit when to work is complete or the
expiry time exceeded. It is the responsibility of the entry supervisor to ensure that a ‘permit to
enter’ has been completed for every confined space entry.

10.0 EMERGENCY ARRANGEMENTS


10.1 Emergency Response Planning and Rescue Procedures
Any attempt at rescuing a person who collapse in confine space should be based on a pre-
arranged plan.

SSSB rescue operation plan should:

• List roles of the designated rescue personnel that are available


• Indicate the methods of rescue to retrieve persons inside a confined space
• Prescribe the types and availability of equipment necessary for rescue
• Provide an effective recall means to summon the designated rescue personnel in a timely
manner

Rescue equipment may include but not limited to:

• Self-contained Breathing Apparatus (SCBA)


• Full body harness with retrieval line attached
• Ladder
• Explosion-proof lighting
• Stretcher
• Approved head protection
• Reviving apparatus
• First aid equipment etc

Ropes, harnesses, fall arrest gear, lifelines, lifting equipment, first aid equipment, protective
clothing and other special equipment is provided for use in case of an emergency.

11.0 AIR VENTILATION


It is important to ensure that the air moving device / air blower is placed where the air is drawn
into the confined space from a contaminant-free source. For example, it is not appropriate to
place the air moving device behind a diesel generator where the exhaust gas of the generator
could be drawn into the confined space.

Note:
For a continuous release of contaminant into a confined space, the forced ventilation N (m3/min)
required to dilute the contaminant (molecular weight) which is generated at a flow rate of Q (cfm),
to a permissible exposure concentration. The volume of confine space is V (L X W X H) and divide
the number 60 by the time in minutes.

Where:
N = number of air changes per hour
Q = Volumetric flow rate of air in cubic feet per minute (cfm)
Vol = Space volume L × W × H, in cubic feet

In the case of a room not being simply rectangular, section off the space into smaller rectangular
spaces mentally. Calculate the volume of these smaller segments, and add the volumes together
to find the total room volume.
