JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
WE, MERLY DELA CRUZ JUNIO and FILIPINAS MATEO
ASUNCION, both of legal age, Filipino citizens, and residents of Barangay
Santa Maria, Camiling, Tarlac, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. We both personally know ALFREDO TIANO ANDRES (Alfredo, for

brevity), who was born on December 18, 1950 to Spouses Bernardo
Andres and Regina Tiano in Camiling, Tarlac;

2. Alfredo is a member of the Social Security System of the Philippines

until his death on January 1, 2018;

3. When Arlyn Andres Agustin, the daughter of Alfredo went to the SSS
Camiling Branch to file for the Funeral Claim Benefit of her late father,
she was required to submit copies of Alfredo’s Birth, Marriage and
Death records;

4. Arlyn was able to secure a CRS Form No. 5 – Advisory on Marriages of

Alfredo from the Philippine Statistics Authority (PSA);

5. After the submission of the required documents, it was discovered in the

Certificate of Marriage of Alfredo that the recorded name of his mother
is REGINA TIANIO instead of REGINA TIANO;

6. While, in the Certificate of Death of Alfredo his name was recorded as

ALFREDO TIAÑO ANDRES, instead of ALFREDO TIANO ANDRES.
Also, the recorded name of Alfredo’s mother appearing therein is
REGINA TIAÑO, instead of REGINA TIANO as recorded in the said
Certificate of Birth and CRS Form No. 5 – Advisory on Marriages;

7. In addition, the recorded name of Alfredo as recorded in his SSS record

is ALFREDO TIANIO ANDRES, instead of ALFREDO TIANO
ANDRES. Hence, the discrepancies;

8. We are not related to Alfredo by affinity or consanguinity except that of

being his barangay mates;

9. We are executing this Affidavit to attest to the fact that the names
REGINA TIANO, as the mother of Alfredo Andres or Alfredo Tiano
Andres as appearing in the said Certificate of Birth and CRS Form No. 5
 –Advisory on Marriages, REGINA TIANIO, as the mother of Aldredo
Tiano appearing in the said Certificate of Marriage, and REGINA
TIAÑO, as the mother of Alfredo Tiaño Andres appearing in the said
Certificate of Death, refers to one and the same persons, and that is the
biological mother of Alfredo, our barangay mate;

10. Also, We are executing this Affidavit to attest to the fact that the
names ALFREDO ANDRES as appearing in the said Certificate of
Birth, ALFREDO TIANO ANDRES, appearing in the said Certificate of
Marriage and CRS Form No. 5 –Advisory on Marriages, ALFREDO
TIAÑO ANDRES, as recorded in the said Certificate of Death, and
ALFREDO TIANIO ANDRES, as appearing in his SSS records, refers
to one and the same person, and that is our barangay mate, Alfredo
Tiano Andres;

11. Finally, we are executing this Affidavit to attest to the truth of the
foregoing statements and for all legal intents and purposes it may
serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this
