1. This Contract is entered into by and between the undersigned parties subject to the following
terms and conditions:
Probationary Employment: {Start Date} to {End Date – up to 180 calendar days only}
Standards/Criteria: The passing rate for regular employment is 80% based on: 30%
Performance, 20% Character Behavior, 20% Teamwork, 20% Attendance , 10% Attendance.
2. The employee agrees to comply with the provisions in the Company Policies and Regulations,
which are incorporated herein by reference.
3. The Employee hereby agrees to deductions authorized by law, including but not limited to,
withholding tax, SSS, Philhealth, Pag-IBIG, debts or cash advances owing to the Employer, and
all other analogous deductions.
4. The Employee shall submit a written resignation letter at least 30 days before the last day of
employment. Within the period, the Employee shall ensure proper turn-over of company
properties and transfer responsibilities to the assigned personnel who shall takeover. Failure to
comply with this clause shall be a ground for non-acceptance of the resignation, as well as a
claim for damages against the Employee. No immediate resignation shall be accepted.
5. The parties hereby agree that this Agreement constitute the entire employment contract and
supersedes all prior arrangements, whether verbal or written.
