Digital Storytelling: Math Story

You will be creating a digital story using a math concept learned this year. This story
must have a purpose or main idea while using a math concept. This story must follow
and include everything in the guidelines, checklist, and rubric. For more information look
at the directions and guidelines for the project.

You will be expected to hand in part of the project on specific due dates. I will be signing
that I review that piece of the project on the given date on this form. There will be
deductions on your final project for missing pieces and not handing in any given piece of
this project on or before the given due date. Each day a piece is late you will lose 2
points. After a week the piece will no longer be accepted with resulting losing points off
the final project. This paper cannot be lost.

Your parent/guardian will have to sign this form, also a part of your project just so they
are aware that you will be working on this project and there is multiple due dates.
