AFFIDAVIT OF DECLARATION

I, ARNALDO P. ARDIENTE, of legal age, married, resident of

Purok Zone 1, Brgy. Bagong Silang, Iligan City, after being sworn in
accordance with law hereby depose and say:

1. That I am one of the children of Julio Ardiente and Helena

Plaza Ardiente;

2. That my father and mother died on 1984 and 1965

respectively;

3. That I knew of their deaths because I was still with them at the
time of their death;

4. I am executing this affidavit to attest to the fact of death of my

parents aforementiones.

IN WITNESS WHEREOF, I have hereunto affixed my signature
