5th May 2013 POLICE COMPLAINT FORMAT FOR CREDIT CARD FRAUD

Subject: Misuse of personal data for fraudulent activity and cheating by unknown person or gang operating in
New Delhi.

Sir,

I am bearer of Citibank Credit Card number 43***8***1829*** since month of March 2009. Today at 3.14 pm when I was
busy at my work, I got an SMS alert on my mobile phone number from Citibank that a telephone Personal Identification
Number (TPIN) had been issued for user of this card without even me requesting for it.

When I checked with customer care center of Citibank at 0124-2542484, I learnt from customer care executive that some
one impersonating me had called up Citibank Credit Card department and has fraudulently got a new TPIN issued. A
TPIN is secret code which can enable a person knowing it to process transaction on credit card.

This shows that some unknown person or gang in an organized manner has fraudulently stolen my personal data or
someone having access to my personal data, that is required for getting TPIN issued, is misusing it for fraudulent
purpose.

Hence I would request you to give your serious attention on this serious issue and take immediate legal action on the
fraudsters involved in this malicious crime of cheating innocent people.

Your in-time action on this complaint can prevent frauds that this person or gang is executing.

Hi team,

This to inform there was two online transactions done with out my knowledge from my Credit card details
Card no.5172526726316297
Name. Rajesh kumar
Amount (19999 rs.) thim 3:17pm
Next (54200) thim 3:20 pm.

Request you to thoroughly invegestigate these two transactions of amount (INR 19999 and 54200) return back to my
account.
