AFFIDAVIT

I, FEDELINA MUTYA LASALA WONG, of legal age, Filipino

citizen, and a resident of Blk 12 Lot 59, Toscana Subdivision, Puan,
Davao City, after having been sworn to in accordance with law, do
hereby depose and say that:

1. I am married to ERNESTO S. WONG last June 3, 1978;

2. My husband went to Malaysia to work as a musician

sometime on 1989. However, despite sending several letters
to his last known address at Malaysia, I received no
response from him;

3. Despite diligent efforts, including asking from my husband’s

relatives and friends, he could not be found;

4. For twenty eight (28) consecutive years, my husband has

been absent and had not been communicating with me, or
his parents;

5. I have already given up hope that I would be able to locate

him again;

6. For all intents and purposes, I already presume that my

husband is already dead;

7. I am executing this affidavit to attest to the truth to all the

foregoing facts and for whatever legal purpose this may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this
